This Is A High-Risk Activity: Unwanted Events: Electrical injury, crush injury, fire/explosion, release of hazardous material
-
Reference Procedures: EIMS-4-9605 Energy Isolation
-
All hazardous energy sources shall be identified and de-energised/isolated prior to working on equipment/systems, with protection of services that cannot be isolated
-
In instances where electrical energy isolation is not practicable, appropriate safe working methodology has been developed and approved, for implementation by competent workers
-
Work planning and controls applied include Isolation of sources of mechanical, pneumatic, hydraulic or gravitational energy
-
A ‘check for dead’ test shall be undertaken prior to working on equipment/systems to verify the equipment/system is in a zero energy state. The ‘check for dead’ test must be conducted by a competent person who is authorised to operate the equipment under 'test' conditions.
-
Competency requirements for personnel required to isolate (AIO) or work on (Protected Person) isolated plant/equipment has been identified and includes: - Trained in EIMS-4-9605 Energy Isolation Procedure
-
Competency requirements for personnel required to isolate (AIO) or work on (Protected Person) isolated plant/equipment has been identified and includes: - isolation equipment as listed in EIMS-4-9605 or site processes
-
Competency requirements for personnel required to isolate (AIO) or work on (Protected Person) isolated plant/equipment has been identified and includes: - confirm isolation (free from harmful energy)
-
Competency requirements for personnel required to isolate (AIO) or work on (Protected Person) isolated plant/equipment has been identified and includes: - development of isolation lists
-
All personnel who are about to commence working on plant or equipment must conduct an isolation check before placement of their Personal Danger Tags /Lock (Function try/test). The check includes either; Physical confirmation isolation lock and tag is in place for field isolations, or Ensuring a Lockbox Officer lock and confirmation tag is in place for Lockbox isolations.
-
Competency requirements for personnel required to isolate (AIO) or work on (Protected Person) isolated plant/equipment has been identified and includes: - placement of Personal Danger Tag / Locks and control group isolations before working.
-
All Authorised Isolation Officers are appointed in writing by the Project/Operations Manager and appointments are recorded. Roles and Responsibilities are clearly outlined in appointment letter.
-
Isolation work instructions are developed, reviewed and approved by Authorised Isolation Officer. Where potentially harmful energy sources are present, the isolation process includes the specific steps for releasing the stored energy to achieve "zero state energy" of other means of controlling potential energy sources
