General Operational Risk Assessment for this Project
DEFINITIONS/ Factors and Considerations Explained: Scope of Operational Risks
-
The purpose of this section is to identify the various hazards that apply to this site. Then to classify the hazards individually, estimate their frequency and designate a hazard strategy to mitigate the individual hazards.
-
The hazards are classified by Class Levels of Class A, B or C
-
"Class A" - A condition or practice likely to cause permanent disability or loss of life or body part and/or extensive loss of structure, equipment or material.
-
"Class B"- A condition or practice likely to cause serious injury or illness (resulting in temporary disability) or property damage at is disruptive, but less than Class A.
-
"Class C" - A condition or practice likely to cause minor (non disabling) injury or illness or non-disruptive property damage.

List of Immediate and Potential Common Hazards and Operational Risk Assessment
DEFINITIONS/ Factors and Considerations Explained: Scope of Operation, Immediate and Potential, Severity, Likelihood and their degrees or factors for each.
-
"Major Scope of Operation" means: ( A "Risk" we may or may not be known and or seldom to come across)
The level of risk is based on Extreme likelihood and severity for the operation as a whole/ Hazards still exists and preventative measure still need to be taken with extreme care! This section may include hazards that are uncommon, unfamiliar and or of very dangerous circumstances/ EXTREME CAUTION IS REQUIRED !! -
"Normal Scope of Operations" means: (A "Risk" that is commonly known/ normal in our line of work.)
The level of risk is based on moderate likelihood and severity for the operation as a whole / Hazard still exists and preventative measure still need to be taken with greater awareness. CAUTION IS REQUIRED! -
"Minor Scope of Operations" means:( A "Risk" that is known but limited in probability and severity/ May not be a direct operational hazard, but a hazard in proximity or association)
The level of risk is limited by likelihood and severity for the operation as a whole / Hazards still exists and preventative measure still need to be taken.
SITUATION AWARENESS REQUIRED. -
Immediate Hazard: A hazard(s) that will be immediately confronted in the project or associated with the project.
-
Potential Hazard: A hazard(s) that is known, but will or maybe be confronted with later in the project or associated with the project.
-
Severity/ Catastrophic: The hazard will endanger the public, hazard may exceed our work area, uncontrollable or very limited control, IDHL (Immediate Danger to Health and Life) is a level that survival will not occur.
-
Severity/ High: Hazard may or may not endanger the public, is at limits of exceeding our work area, limited control, IDHL (Immediate Danger to Health and Life) is a level that survival may or may not occur.
-
Severity/ Medium: Hazard will not endanger the public, is within the limits of our work area, controllable, IDHL (Immediate Danger to Health and Life) is a level that survival may occur.
-
Severity/ Low: Hazard will not endanger the public, is well with the limits of the work area, easily controllable, IDHL (Immediate Danger to Health and Life) is a level that survival will occur.
-
Severity/ Little to No Likelihood: Hazard will not endanger the public, is very localized, easily controllable, IDHL (Immediate Danger to Health and Life) possibility to produce serious harm is not likely, first aid may or may not be necessary.
-
Likelihood/ Extremely Likely: Easily Foreseeable, easily predictable; Probability 90- 100%
-
Likelihood/ Very Likely: Reasonably Foreseeable, Reasonable predictable; Probability 60- 90%
-
Likelihood/ Minimal Likelihood: Not necessarily Foreseeable, not necessarily predictable: Probability 30-60%
-
Likelihood/ Little to No Likelihood: Not Foreseeable, not predictable, very rare odds: Probability 0-30%
