SOT - Use primarily to record items related to behaviour rather than spotting hazards. Example: Notice that A N Other is not wearing safety shoes (Could be a behaviour or lack of PPE). Discuss situation to find out why not wearing them. It may be that the safety shoes are uncomfortable, so solution is to replace them. How & Why people are working in the way that they are. Look to see if persons are aware (their work, surroundings, the affect they have on others and others have on them). Look to see if people are taking ownership of issues or if they ignore them. Look at the physical way in which people work & for awareness of how they may put themselves & others in danger.
