Stop Work Form This form is to record action taken to stop work due to safety issues / concerns. Discuss these concerns with the client and try to resolve successful. Don't Look the Other Way

  • If possible take a picture of the cell / location / area of work / situation causing the Stop Work Action.

  • Describe the Stop Work Situation

  • Add Sketch here if required.

  • Can the situation be made safe?

  • Confirm that you are stoping the work

  • Describe actions to make activity safe

  • Confirm that it is safe to continue work

  • Has a representative of the customer been made aware? If Possible get customer to sign to show agreement to stop work.
    Record any further relevant details, photos etc.

  • Inform Customer. Record details of who you have informed.

  • Customer Signature if Possible

  • Record details of who you have informed

  • Record details of any agreed actions to be taken
