  • 1) This form will be used by the site responsible supervisor or manager to carry out an audit of the behaviour of workers, employees and contractors in respect to HSE issues. 2) Each behaviour audit shall include a comprehensive observation. All items will be recorded on the management system

Yard

  • Correct PPE being worn as per site requirements ?

  • Pedestrians are keeping to walkways. No - one seen taking short-cuts?

  • Pedestrian interaction - Unless vehicles are stationary? Distances adhered to 3m forklifts /10m telehandlers /14m 360 machine

  • Loading/ Unloading Area rules are being followed?

  • Gates shut when not in use?

  • Forklifts driving rearwards where visibility is impaired?

  • Vehicles not operating too fast or driving dangerously ( inc Drivers not using phones ) ?

  • Forklift operators wearing seat belts?

  • Horn being utilised in areas where vehicle / pedestrian interaction may take place

  • Keys removed from vehicles when not in use

  • Forklift checklists carried out - request evidence ?

Employees and contractors

  • Permanent site workers including employees and labour hire are inducted ( Challenge at least 4 ) ?

  • All contractors on site are inducted ?

  • Public Liability Insurance available

  • Applicable licences are available such as Forklift, Electrical, Fitter , High Risk ?

  • Applicable permits filled out and authorised such as hot work and confined space ?

  • Control panels locked and in good condition

  • Are isolation controls in place if required ( check maintenance tasks )

  • Are walking surfaces free from overly large spills

  • Are noise levels within the limits advised and is ear protection provided and being worn?

Phase 4

Training and competence

  • Is training and competence to be inspected

  • Are all operatives inducted on to site

  • Are all operatives trained and competent to carry out their duties

  • Are all statutory licences available and in date? ( pick 3 )

  • Are operatives aware of SSOW and RA to carry out their duties

  • Are all operatives aware of the PPE requirements on site

  • Are TBT's being conducted and signed

  • Are all operatives aware of isolation procedures required and are they being followed

  • Are all persons aware of their statutory duties under the Health And Safety At Work Act 1974

Contractors

  • Have contractors provided RAMS prior to works taking place and have they been reviewed? Specific RAMS to be provided 72hrs before work commences.

  • Have all contractors been inducted on to site and copies made of any appropriate licences?

  • Is an isolation required for the task and is it in place
