Installation Information

 • Is the installation complete?

 • Please give details on what prevented completion (eg are there any special equipment/materials required, is it a two-man installation, is special access required)

 • Describe aerial location, cable route and where cable (TV end) has been terminated and left

 • State which option of cable entry was used:
1. External Wall entry hole – Approx 2200mm high (central to where the TV wall is going, NOT central to full length of wall). No trunking needs to be left on site if the cable is terminated behind where the TV is going.

2. External Wall entry hole – Above the 2400mm height of the new TV wall. Trunking to be left on site for installation by contractor.

3. Cables left in ceiling – Trunking to be left on site for installation by contractor. Main contractor to pull cable through from ceiling. 2000mm tail to be left on cable from point above TV wall.
• Was at least 2 m free length of cable and 2 m trunking left on site for installation and connection by others?

 • Describe where the trunking was left. Need to leave it with the house management staff (GM ideally, if they are not on site then the DGM or duty manager) and explain it will be needed for the upcoming refurbishment project in the play area.

 • Please give details why the installation was not completed as per brief

 • Has signal been tested on meter and all working ok?

 • Describe issues

 • State levels and confirm if all muxes are being received

 • Any additional comments?
