1.0 - General Issues

  • 1.0 - All persons on site have sited and signed the hazard ID and been inducted for the construction site.

  • 1.2 - Suitable provision has been made for toilets , washing and general welfare facilities.

  • 1.3 - All vehicles are parked in the designated car parks and vehicles on site are parked in a safe zone.

  • 1.4 - First aid container in site hut and clearly marked.

2.0 - Site Safe Systems

  • 2.1 - Evacuation plan displayed and understood including the main muster station.

  • 2.2 - Risk assessment statements have been completed and made available to all parties.

  • 2.3 - Fire Extinguishers in place, clearly marked for type of fire and inspection tags are current. Extinguishers clear of obstructions?

  • 2.4 - All persons on site are aware of the Safe Systems to be used and any Permit to Work System.

  • 2.5 - Appropriate barriers and safety signage are in place.

  • 2.6 - The site is tidy,and materials stored away safely.

  • 2.7 - All plant and machinery is right for the job, in a safe condition and staff are adequately trained and have been checked for competency for plant he or she is operating.

  • 2.8 - Every one on site is wearing sufficient PPE, the gear is in good safe condition including safety boots, high visibility vest or jacket hard hat ETC.

3.0 - Vehicles on Site

  • 3.1 - Good natural lighting?Separate pedestrian and vehicle routes are clearly marked.

  • 3.2 - Contractors vehicles are parked only in a good and safe location.

  • 3.3 - Vehicles and machinery on site are fitted with reversing warning alarms.

4.0 - Electricity

  • 4.1 - All power leads and power tools have current electoral tested tags fitted.

  • 4.2 - RCD are to used where needed.

  • 4.3 - Generators are to placed away from trenches and man holes or any area the fumes from motors my affect any worker.

  • 4.4 - All electrical equipment is stored away safely and in a dry place.

  • 4.5 - Power tools in use are in good working order and fit for task.

  • 4.6 - Electoral tools are to be only used by any one that has been trained in competency for that device.

  • 4.7 - All power tools faulty are tagged for repair.

  • Is there a current power permit

5.0 - General Equipment.

  • 5.1 - Clear of rubbish?

  • 5.2 - Tools not in use kept in safe area as not to create trip hazard

  • 5.3 - Any damaged hand tool are taken off site as not to be used and are able to be repaired.

  • 5.5 - Work height correct for the type of work and the employee?

  • 5.6 - Rubbish Bins located at suitable points around site?

  • 5.7 - Rubbish Bins emptied regularly?

  • 5.8 - Are footpaths in good condition?

6.0 - Protecting The Public.

  • 6.1 - All work areas are clearly securely segregated.

  • 6.2 - Are footpaths in good condition?

  • 6.2 - All adjacent public access areas and walkways are level and free from trip hazards, obstructions such as stored material and waste.

  • 6.3 - Emergency Escape routes are not compromised.

  • 6.4 - Work area is fenced off securely from public access.

  • 6.5 - Is there adequate signage for the public. Ie: Hazard ID board, foot path signs, Forman on site contact number etc

7.0 - Machines

  • 7.1 - Operators are licensed, trained and have competency for the equipment they are operating.

  • 7.2 - Machine is well maintained and has current service sticker.

  • 7.3 - Machines are tidy and have no obvious faults or damage.

  • 7.4 - Machines fitted with reversing warning device.

  • 7.5 - Is the correct PPE being used. Ie: hearing protection, eye protection, foot protection, etc

  • 7.6 - Are there working rotating beacons, LED, strobe etc warning devices on top of machines?

8.0 - Excavations

  • 8.1 - Is excavation greater than 1.5 meters deep?

  • 8.2 - If excavation is deeper than 1.5 deep is there shoring ? Or batters cut back to a safe slope.

  • 8.3 - If excavation is dealer than 1.5 meters has work safe N. Z been notified.

  • 8.4 - Is there a safety pin or secondary locking device on quick hitch on excavator to prevent attachments from falling off.

  • 8.5 - Is spoil placed at least 600 mm from edge of trench?

  • 8.6 - Has machines had there pre-start check sheet filled out and is current?

  • 8.7 - All lifting chains have current certification.

  • 8.8 - Is there a spotter being used ?

  • 8.9 - If excavating near power cables either over head or under ground, are there current power permits?.

  • 8.10 - Noise levels controlled?

  • 8.11 - Operators trained/inducted into the operation of the machines?

Lifting Equipment

  • All lifting chains on site have current certification tags.

  • Any swift lifters or lifting clutches have current certification tags.

  • All lifting chains have hooks with safety catches or equivalent safety device.

  • Hooks, lifting eyes or chain shortening devices are attached to chains or rings by way of roll pins not split pins.
