Post Contract Evaluation Questions

  • 1. Was the contract completed on time?

  • 2. Was the contract completed on budget?

  • 3. Did the contractor take all necessary steps to avoid disruption to the area?

  • 4. Were there any hazards/risks generated by the Contractor?

  • 5. If ‘Yes’ to Q4, were they adequately controlled?

  • 6. Was the risk properly managed and documented on the correct risk assessment paperwork?<br>• Permit to Work (PTW) for Hot Work / Work at height / ‘Live’ electrical work (fault-finding)<br>• JSA – for any other medium-to-high risks work

  • 7. Did the contractor(s) wear the required PPE for the tasks?

  • 8. Has there been any safety or environmental incidents (including near-miss) during the contract, including causing injury or near-miss to other persons?

  • 9. Were these incidents and/or near-miss reported by the contractor to Your Company?

  • 10. Did any of the reported incidents or observations suggested poor H&S practices by the contractor?

  • 11. Did all contractors always adhere to the site rules? E.g. ‘no-smoking’, speed limits, using walkways, PPE, signing-in, etc.

  • 12. Did the contractor report any new hazards that they were exposed to whilst completing the work that were not considered prior to the work? Note: Update hazard registers.<br>

  • 13. Did all the contractors complete Your Company contractor Induction prior to working?

  • 14. Did all contractors all have the required licenses or training certificates for the type of work they were completing? Examples.<br>• Asbestos removal license for any quantity of ‘friable’ asbestos or over 10m2 of ‘non-friable’.<br>• NZQA Forklift training (and license ‘F endorsement’ if using public areas)<br>• Digger / loader / roller / hopper - operator training<br>• NZQA Confined Space entry training, gas detectors; rescue equipment & plans<br>• NZQA Heights training if using harnesses, mobile elevated platform, scissor lifts<br>• Welding training certificate<br>• Electrician registration<br>

  • 15. Overall, how would you rate the performance of the contractor based on these and other recent observations or dealings? Score 1 to 5; (1=Poor – 5=Excellent)
