Section 2 - Emergency Action Plan (EAP)

Has a current site specific emergency action plan been written that addresses:<br> <br> * All emergencies deemed appropriate and reasonable such as man-made and civil emergencies, natural disasters, environmental incidents and workplace violence?<br> * Identified by name emergency preparedness coordinator?<br> * Line of Authority?<br> * Emergency response teams (chemical spill, mill rescue, etc.)<br> * Emergency response first aid teams (if the response times from outside professionals exceeds 15 minutes.)<br> * Emergency utility and process shut down procedures.<br> * Emergency communication.<br> * Evacuation routes and procedures.<br> * Evacuation list of employees prepared and current.<br> * Evacuation list of visitors and contractors.<br>

Has an emergency preparedness drill (hands-on or table-top) been conducted and critiqued in the last 12 months on each shift of operation?<br><br>* The drill must involve management, supervision, employees and the plant’s emergency response teams."<br>*The drill must be based on a realistic disaster or emergency scenario to be managed (for example: AED response, roll-mill incident, chemical spill response, etc.)<br>*A documented critique is required; follow up actions tracked to completion.<br>* At a minimum, there must be an annual facility evacuation drill involving the entire site personnel.<br><br>Note: Each facility will conduct a planned or unplanned drill of some aspect of the Emergency Action Plan.<br>

  • The auditor will meet with one emergency response coordinator and one other key member of an emergency response team and evaluate their ability to execute a randomly selected scenario from their plan. Team leaders and members are expected to communicate their planned response to the scenario. Actual execution is not required.

    INTERVIEW QUESTION: Describe a response to a large volume release of hazardous chemical. Describe the steps taken in an extreme weather condition (tornado, flood, monsoon). What are the planned responses to an explosion?
