Procedure

  • Is there a written and approved site procedure for working at height?

  • Has it been communicated to the employees and contractors?

Working at height

  • Have employees and contractors been trained on working at height?

  • Has a risk assessment been conducted and signed off before starting work at height?

  • Has a permit to work been completed and approved?

  • Has fall prevention been implemented wherever possible?

  • Are all workers at height wearing fall protection connected to secure anchor points?

  • Has the area below the work at height been barricaded off?

  • Have tools and loose equipment at height been secured to prevent it from falling?

Ladders

  • Is there a register for ladders?

  • Are ladders inspected before use to ensure there are no missing rungs and that they have rubber footing?

  • When used are ladders tied off at the top to a secure anchoring point or otherwise supported by a person on the ground?

  • Never use alluminium or conductive metal ladders within 6 meters of any live overhead electrical powerline.

Scaffolds

  • Are scaffolds erected and dismantled by trained scaffolders?

  • Are scaffolders wearing fall protection while building or dismantling the scaffold?

  • No scaffolds erected in close proximity to live overhead electrical power lines

  • Are all scaffolds level, secure, stable, and properly braced/tied (secured to permanent structure)?

  • Are all components if the scaffolds adequately secured to prevent displacement?

  • Do all scaffolds have a handrail and fixed ladders (not step ladders)?

  • Have scaffolds been inspected by a qualified scaffolder before use and is there a tag that certifies safe to use?

Mobile elevated platforms

  • Are employees working on mobile platforms wearing fall protection?

  • Are operators of elevated mobile platforms aware that they cannot move with people in it?
