Fall Protection

  • If needed has fall protection been inspected for damage prior to use? (Safety Harness, Safety Lanyard, Anchor Points)

  • Correct ladder selected, inspected and, used? (A-frame, extension ladder, trench ladder, step ladder)

Housekeeping and Restrooms

  • Restroom clean?

  • Hand wash station and trash can with restroom?

  • Are restrooms within 1/4 mile or a 5 minute walk?

  • Is the Restroom to worker ratio correct? (15 people : 1 restroom * if a female is on the crew a female restroom is REQUIRED and doesn't count for the overall ratio)

Unsafe Acts or Conditions
