    Should the officer have any concerns the CHSCD Manager and Contracts Manager must be informed verbally as soon as practicable. The form MUST be completed regardless. Failure to do so may result in performance bonus payment being withdrawn.

  • Are the guardrails and toe-boards installed and secures? (Absolute requirement at working platform heights greater than 1.2m)

  • Is the working platform board correctly located with the access hatch adjacent to the internal ladders or staircase?

  • Has the tower scaffold been erected at safe distance from overhead cables (<4m)?

  • If the tower is to be left incomplete, has a sign been displayed warning that it is unsafe to climb?

  • If extra stability is necessary can the tower scaffold be tied to a neighbouring structure?

Completion of Works Declaration
