I have had the relevant method of works statement, risk assessment(s) and toolbox talk read to me by the site supervisor and I fully understand the safe systems and methods of work to be adopted by myself during the project.
I will not attempt under any circumstances to undertake any duty for which I have not been suitably trained or instructed in how to carry out safely. IF UNSURE I will seek advice from supervisory staff before attempting to carry out any such duty.
I fully understand that I have a personal legal duty under the 1974 Health & Safety at Work Act to ensure the health and safety of: my-self and any other person who may be affected by my actions. I also understand I may be prosecuted in a court of law for any such breaches in legislation. -
