  • This form is to be used by field staff to assess general site safety issues prior to commencing work activities. It is to be used in conjunction with relevant generic risk assessment. Please tick where applicable on the following questions.

  • Are there any health and safety incidents on site that require reporting? <br>If so please call the office.

  • Are the activities to be carried out controlled and without any significant risk to safety?

  • Are you in possession of all the relevant documentation required for the activities you will be carrying out? (Plan of work, Risk Assessment etc.)

  • Is there a facility to park your vehicles safely without obstructing others?

  • Is the access to the site safe and without risk if injury to yourself or colleagues?

  • Are other contractors on site working in a safe manner?

  • Have all ladders or steps that you will be using been inspected and indicated with the required colour code?

  • Has any scaffolding been erected safely and been inspected with the last 7 days?

  • Has all electrical equipment you will be using been PAT tested and within test dates?

  • Is all machinery and equipment suitable for the task and in a safe condition?

  • Have all operatives been sufficiently trained in the use of tools and equipment to be used?

  • Have toolbox talks been undertaken?

  • Have any risks of people falling from height been sufficiently controlled?

  • Have all live electrics, gas, or heating pipes been isolated (where practicable)?

  • All groundwork's / excavations - are cable records, specific risk assessments, specific standard working procedures on site and available to you before work commences?

  • Is there access to suitable and sufficient personal protective equipment RPE / PPE?

  • Have all substances hazardous to health been identified and suitable controls installed?

  • Have all other aspects that may have an effect on safety been adequately identified and controlled?


  • IF ANY OF THE 'NO' BOXES HAVE BEEN TICKED THE WORKS MUST NOT PROCEED AND YOU SHOULD INFORM YOUR SUPERVISOR OR MANAGER IMMEDIATELY.
