  • We have provided this questionnaire to assess the risk to employees who use VDU's for more than one hour per day as part of their duties. This questionnaire will be kept and reviewed as part of the risk assessment process and complies with the Health and Safety (Display Screen Equipment) Regulations 1992

Training and Information

  • Do you understand the risks of display screen equipment?

  • Do you know how to adjust your work station?

Posture

  • Can you sit comfortably and easily change your posture?

  • Can you adjust your equipment to a comfortable viewing position?

  • Can you place your feet firmly on the ground?

Workstations and Surfaces

  • Have you arranged the workstation to suit your specific requirements?

  • Is there sufficient space in front of the keyboard to support your hands and forearms?

  • Do you have sufficient legroom

  • Is your workstation and surrounding area free from obstructions and hazards?

  • Does most of your work require you to read from hardcopy documents?

  • If yes, do you require a stand or holder?

Display Screen

  • Is the information displayed on your screen clear and easy to read e.g. text size?

  • Do you know how to adjust the text size?

  • Can the brightness and contrast be adjusted easily?

  • Is your station free from reflective glare?

  • Is the image on the screen , stable and free from flicker?

  • Does the monitor swivel adequately in each direction?

  • Are window coverings available to use?

Keyboard

  • Is the keyboard separate from the screen?

  • Can the tilt of the keyboard be altered/adjusted to find a comfortable typing position?

  • Are the key symbols easy to read?

  • Is there enough space to rest the hands in front of the keyboard?

Mouse

  • Is the device suitable to task?

  • Is the device positioned close to the user?

  • Is the users wrist supported?

  • Does the device run smoothly?

  • Does the user know how to adjust the settings for speed and accuracy

Furniture

  • is the chair comfortable and can the height and backrest be adjusted?

  • Can all adjustments be made easily?

  • Is the small of the back supported?

  • Is the desk space large enough for all equipment and space to manoeuvre?

  • Is the chair stable?

  • Is the top of the VDU roughly the height as the eyes?

  • Are the feet flat on the floor without too much pressure from the seat?

Environment

  • Has your equipment been situated to avoid direct glare?

  • Does the light allow you to work comfortably?

  • Are all levels of noise, light and heat comfortable?

  • Can the above be adjusted?

  • Does the air feel comfortable?
