  • Have all personnel involved in the task read and signed onto SWMS?

  • Has a pre-start inspection on the EWP been completed for the day?

  • Has operator of EWP had the correct training, i.e. Scissor Lift (SL), 11m boom type HRWL (WP)?<br>

  • Are employees in the EWP basket working at heights trained?

  • Are personnel on boom type EWP wearing safety harness and hooked on at all times?

  • Has an exclusion zone been developed to prevent unauthorised access?

  • Is the EWP placed on level and sturdy ground?

  • Are underground services identified where the EWP is going work and travel path free from obstruction, level, and safe for travel? (Uneven ground and underground services, i.e. pits, may cause operator ejection)

  • Is there sufficient clearance between any part of the EWP and powerlines or overhead structures?

  • Is the fire extinguisher mounted on the machine and in date?

  • Are employees facing the direction of travel and basket with 2m while the EWP is in motion?

  • Is the working load limit (WLL) for the EWP clearly visible and not exceeded?

  • Is the working platform in good condition and free of rubbish?

  • Is the operator lowering or raising the EWP without leads/cords/hoses tied off to the basket?

  • Are outriggers packed/supported if they have been extended?

  • Are all the controls marked with appropriate functions (both basket and ground controls)?

  • Is a spotter present and guiding the operator?

  • Can the spotter explain the process of lowering the machine to the ground in an emergency?
