  • Are individuals aware of any limitations of the permit?

  • Is there a risk assessment and method statement for carrying out the works? (Management of Health and Safety at Work regs 1999- Section 3.)

  • Are those doing the job aware of the contents of the risk assessment and method statement and signed on to it?

  • Can those going to do the job demonstrate competence to do the task effectively?

  • Do those carrying out the work understand how the activity is to be carried out effectively and safely?

  • Is the risk assessment and method statement being followed correctly? (HASAWA 1974 - Section 7)

  • Is the general housekeeping of work area to a high standard

  • Have any additional hazards within the work area been identified and made safe?

  • Have all floor openings, voids been correctly barriered and made safe? ( Work at Height Regulations 2005- Section 11))

  • Are lifting operations being carried out? (LOLER 1998)

  • Has the area been sufficiently barriered?

  • Are the individuals trained and competent for carrying out lifting tasks?

  • Has a lifting plan been issued for the works?

  • Is all lifting equipment in good condition and coded "in date"?

  • Have all individuals been trained and are competent to operate any tools/equipment?

  • Are all tools, and equipment to do the job appropriate and in good condition? (PUWER-1998 - Section 5)

  • Are all relevant tools portable appliance tested (PAT)? ( Electricity at Work Regulations 1989/ PUWER 1998)

  • Are all relevant tools calibrated and in date?

  • Are the hot works being managed effectively?

  • Is correct PPE being worn to carry out hot works?

  • Is the area free from flammable substances?

  • Is there a confined space permit in place? ( Confined Space Regulations 2005 - Section 4)

  • Are the individuals trained and competent to work in a confined space? ( Confined Space Regulations 2005 - Section 4)

  • Has a safe atmosphere test been carried out by a competent person and is in date? ( Confined Space Regulations 2005 - Section 4)

  • Is a confined space rescue plan in place? ( Confined Space Regulations 1997 - Section 5)

  • Is waste being effectively and properly managed? ( Waste Regulations England and Wales 2011)

  • Are dust emissions being controlled effectively? ( COSHH Regulations 2005)
