Dynamic Risk Assessment

General Site Hazard Identification

  • Presence of other streetworks/ construction activities in the inmediate surrondings.

  • Lone Working

  • Adverse weather: Thunderstorms, Lightning

  • Are you working aloft poles and/or near power lines?

  • Adverse Weather: Rain, Snow and/or Ice.

  • Adverse weather: Winds. Check the wind spead against the Beaufort Wind Scale

  • Is wind above the Level 5?

  • Are you wroking at heigh, using ladders or Cherry Picker?

  • Are trees or loose objets in the vicinity?

  • Insufficient level of lighting (Working In Hours Of Darkness)

  • Un-cooperative residents or members of the public/ thieves.

  • Traffic and/or pedestrians.

  • Is your vehicle parked safely?

  • Are you able to keep the strictly minimum 1 meter footway width?

  • Are you able to provide a safe alternative route for the pedestrians or Traffic Management available?

  • Surface conditions/ ground stability: uneven surface, trailing leads and cables, spillage of oils, mud or and gravel.

  • Additional control measures required?

  • Detail additional controls.

Work UG hazard identification

  • Do you undertake Work in the PIA Underground?

  • Medium Risk/ High Risk Confined Space

  • Poor structure condition of UG infrastructure access (structural integrity, chamber collapse, Joint box/frame & cover defects, corrosion of steps...)

  • Presence of utility service other than telecommunications (electricity, gas, water)

  • Presence of flammable or noxious gas in the Joint boxes of UG structure.

  • Presence of asbestos.<br><br>***In PIA UG the Openreach asbestos register must be scanned in the specific areas before any activities start. Communicate with your Line Manager if you are not aware of this information.

  • Is the asbestos disturbed or the activity could disturb the asbestos?

  • Presence of Pest/ Vermin.

  • Presence of Sharps and/or neddles

  • Presence of water/mud in chamber

  • Additional control measures required?

  • Detail additional controls.

Work at height hazard identification

  • Is it work at height necessary?

  • Do you work aloft poles?

  • Presence of foliage, tree braches or insect nest dificulting the access at height

  • Presence of creosote at the pole.

  • Installed dropwires with less thant 5.2 meters of clearance across a carriageway.

  • General status and stability of the pole.

  • Confirm that you undertake a Pole Pre-Work Check as per instructions above.

  • Is the pole safe to work aloft?

  • Presence of Overhead Power Cables

  • Do you undertake Work at height using ladders?

  • Do you work using MWEP?

  • Is the MWEP operative Accredited? (IPAF 1B)

  • STOP WORK. MWEP can only be operated by trained and competent personnel. Communicate to your line manager.

  • MWEP Operator confirm that have reviewed and comprehended the Risk Assessment Method Statements (RAMS) pertinent to the activity he/she is engaged in. MWEP Operator pledge to apply the control measures outlined in these documents on the scope of he/she activity, along with any extra safety measures determined by the Dynamic Site Risk Assessment of the workplace that he/she have conducted and for which he/she hold full responsibility.

Customer Premises/ MDU Hazard Identification

  • Do you undertake work in MDU/Customer Premises?

  • Drilling activities: Dust exposure

  • Drilling activities: Noise exposure

  • Drilling activities: Vibration exposure

  • Utilities build in customer in-wall? (electric installation, plumbing, gas pipe)

  • Working in a confined space?

  • Environmental hazards/Bees/Wasps/Vermin or hazardous substances

  • Presence of Sharps and/or neddles in the surrondings?

  • Additional control measures required?

  • Detail additional controls.
