1 - FRA Suitability & Sufficiency. - A new FRA should be undertaken if the answer to any of the below questions is NO.

  • 1.01 Is the original FRA available, Current<br>and does it cover all areas within the<br>premises?

  • 1.02 Do all working practises remain<br>unchanged including the way they have<br>been undertaken or organised, including<br>disabled evacuation procedures that are<br>not adequately controlled?

  • 1.03 Is the building free from any significant<br>alterations both internally or externally,<br>structure, layout or storage since the last<br>review?

  • 1.04 Are all fixtures, fittings and materials<br>unchanged since the last review /<br>assessment with no alterations that<br>significantly increase the risk of fire?

  • 1.05 Do all storage areas remain the same<br>with no change of use, or increase in the<br>storage of hazardous substances since the<br>last review/assessment?

  • 1.06 If the number of persons working in<br>and/or using the premises has increased<br>since the last review/assessment, are the<br>additional risks adequately controlled?

2 - Administration Documentation & Certification.

  • 2.01 Have all action points from the<br>previous FRA/review been actioned?

  • 2.02 Is the fire safety due diligence<br>documentation being completed and kept<br>up to date within agreed time scales?

  • 2.03 Is the Health & Safety policy manual<br>available (section 9 Fire)?

  • 3.1 Is there a fire plan off the premises<br>displayed on a wall/notice board (detailing<br>exits, extinguishers, emergency lighting,<br>call points etc)?

3 - Signs and notices.

4 - Fire Training.

  • 4.01 Have all employees been inducted in<br>fire safety (El-box day 1)?

  • 4.02 Have all employees received regular<br>(in line with company policy) refresher fire<br>training?

  • 4.03 Have periodic fire evacuation drills<br>been completed (in line with company<br>policy) ?

  • 4.04 Have employees been trained to<br>evacuate people with different disabilities?

  • 5.04 Have all fire extinguishers been<br>serviced in the last 12 months ?

  • 5.05 Is all other fire fighting equipment (e.g.<br>Annul system, hoses, sprinkler systems etc)<br>In a good operational condition ?

6 - Fire detection, Alarms and Emergency Lighting

  • 6.01 Are emergency lights in good<br>operational condition and working<br>correctly?

  • 6.02 Is the fire panel operational and<br>showing no faults?

  • 6.03 Are all Smoke/heat detectors in good<br>condition and functional?

  • 6.04 Are all call points in good condition<br>with all break glass intact and signed with a<br>completed fire action notice?

  • 6.05 Are all call points numbered, tested on<br>a rotational basis and is the testing key<br>available?

  • 6.06 Is the method for recording faults in<br>place and have all required actions been<br>undertaken?

  • 6.07 If magnetic holders are fitted to fire<br>doors are they tested regularly to ensure<br>their correct operation?

7 - Fire doors

  • 7.01 Are all frames and doors in good<br>condition and operational with no obvious<br>gaps that effect their functionality or<br>integrity?

  • 7.02 Have all fire doors been fitted with self<br>closing mechanisms and are they working<br>and in good operational condition?

  • 7.03 Are all door smoke seals/intumescent<br>strips in place and in good operational<br>order (e.g. Not painted over etc) ?

  • 7.04 Do all fire doors have the correct<br>mandatory signage displayed on them?

  • 7.05 If any fire door is signed " keep<br>locked" is appropriate action taken?

  • 7.06 Are all fire doors securely closed and<br>not wedged open?

  • 7.07 Where a fire door has a glass panel<br>does it have fire glass fitted (bs476<br>compliant) ?

  • 7.08 Are all fire doors appropriate to their<br>location?

8 - Fire Exit Routes

  • 8.01 Are all routes to a place of safety, (<br>assemble point) unlocked, unobstructed<br>and with no other hazards (trip hazards,<br>water, ice etc) ?

  • 8.02 Are all emergency exit doors in good<br>condition with crash bars or other opening<br>mechanisms free from defects and easy to<br>open?

  • 8.04 Are appropriate areas signed keep<br>clear to ensure exit routes remain<br>unobstructed?

  • 8.05 Is there an assembly point of suitable<br>location and size?

9 - Kitchens

  • 9.01 Is all cooking equipment clean and<br>free from significant amounts of grease and<br>cooking debris?

  • 9.02 Are all extraction canopies in a clean<br>condition and free from significant amounts<br>of grease and cooking debris?

  • 9.03 Are grease filters in place and free<br>from significant amounts of grease build up<br>and of a good operational condition?

  • 9.04 Are all gas and electric cut off points<br>easily accessible and signed?

  • 9.05 Are there any other hazards that<br>represent a fire risk in the kitchen area that<br>have not been highlighted and adequately<br>controlled?

10 - Back of house and storage areas

  • 10.01 Are all back of house storage areas<br>free from significant fire risks (build up of<br>combustibles, flammable a etc)?

  • 10.02 Are all high risk materials<br>(flammables, pyrotechnics etc) stored in<br>appropriate areas away from sources of<br>ignition?

11 - Boiler and Plant room

  • 11.01 Are all boiler rooms and plant rooms<br>kept locked with appropriate signage?

  • 11.02 Are there any items stored in plant<br>rooms or boiler rooms that are combustible<br>and fuel the spread of fire?

12 - Other hazards

  • 12.01 Have all other fire hazards that have<br>been observed adequately controlled?

  • 12.02 Has access been gained to all areas<br>in order to undertake the review?

  • 12.03 Have any buildings or business within<br>the locality that have had building works or<br>increased capacity, been taken into<br>account with regards to fire risks and/or fire<br>safety?

  • 12.04 If the premises has adjacent<br>residential accommodation, has there been<br>sufficient provision in the fire evacuation<br>procedure to take into account associated<br>risks and to alert sleeping<br>occupants/residents?
