First Aid Equipment Check

  • It is important to regularly check first aid equipment to ensure that items have not expired and that there is adequate stock available. The contents of first aid boxes must be checked weekly using this form. The functionality of the AED should also be checked weekly and recorded using this form.

  • Please select the First Aid Equipment being checked.

Reception First Aid Box

  • The Reception First Aid Box should contain the following items when fully stocked.

  • x1 First Aid Guidance Leaflet

  • Washproof Plasters x40

  • Sterile Eye Dressings x4

  • Triangular Bandage x6

  • Powder Free Nitrile Gloves (Pair) x2

  • Safety Pins x12

  • Sterile Dressings 12x12cm x9

  • Sterile Dressings 18x18cm x3

  • Alcohol Free Cleansing Wipes x10

  • Is there a suitable amount of stock available in the first aid box and are contents within expiry date?

  • If any of the items require replenishment, please check the first aid cabinet in the first aid room for available stock to replenish. If there is not enough stock available in the first aid cabinet, please report to your line manager. Please dispose of expired items and replace these with new stock from the first aid cabinet. Once replenished, you may adjust your answer to the original question accordingly.

  • Were you able to replenish the missing/expired stock?

AED (Held at Reception)

  • It is important to check the functionality of the Automated External Defibrillator (AED) to ensure that it will be able to function properly in the event of a real emergency.

  • To conduct the check, please remove the AED from it's case and check the battery icon on the small screen. Ensure that the battery icon is displaying a full charge. You may power on and power off the AED to check that functionality appears normal

  • Check the pads on the rear of the AED to ensure that they have not expired.

  • Is the AED fully charged and are the pads within their date of expiry (by at least 3 months)?

  • What is the issue?

  • Please report this issue immediately to your line manager.
