Have there been communication to relevant personnel regarding the forklift policy?

Has a Forklift Daily Checklist (Form F12-01) been completed in full for each of the forklifts in operation prior to their use?

Have corrective action been identified and implemented for all check list items identified as failed?

Are all forklift driver wearing the seat belts fitted to the forklifts?

Do all forklifts have working reversing beepers?

Do the forklifts have working and visible reversing lights?

Do the forklifts have drivers seat that are not damaged and are in good condition?

Do the forklifts have side/reversing mirrors that are not damaged and are I good condition

Do the forklifts footsteps have nonslip surface applied to them which are not damaged and are in good condition?

Do the drivers of the forklifts know the maximum sipped limit allowed on the worksite?

Are the forklifts observed to be driving at the speed limit?

When parked, are the tines of the forklift resting on the ground and the hand break applied?

When not in use, do the forklifts have their keys removed from their ignition and placed in a secure location?

For forklifts operating with a jib or attachment :-<br>- Is the jib chained/pinned to the forklift?<br>- Is the finger bar secure on the attachment?<br>- Does the jib/attachment have a manufacturers compliance tag affixed?

Does the company have on file a copy of every drivers Forklift Certificate of Competency?

Do the forklifts have a rating plate fixed to the bodies and clearly readable?

Is there evidence to support regular inspections and maintenance by qualified technicians?

Are pedestrian walkways clearly marked and complied with?

Does signage exist which details the speed limits and rights of way of forklifts and pedestrians?

Are site traffic rules on display at all points of entrance to site/work area?

During loading and unloading of trucks does the driver of the truck remain in the safe zone, either in the cabin or at the trailer/tray end of the truck with a 3 meter radius from the forklift?

Does the loading and unloading continue if any person is not located within the safe zone?

As part of your general observations have forklifts been observed being driven :-<br>- too quickly for the working environment?<br>- with raised tines?<br>- forward with drivers view obscured?<br>- around corners too fast?<br>- racing other forklifts, vehicles , pedestrians etc?

During forklift refuelling, we're the forklifts observed to have their engines turned off and hand breaks applied?<br>Was the hose securely connected to the tank?<br>We're the drivers aware of the function of the deadman switch?
