  • Risk Scope:
    Fall from Height includes single person scenarios resulting from fall from unprotected edge of highwalls, lowwalls, slopes, crests, active faces, trenches and drains.

  • Control Objective:
    Controls are in place to prevent access and exposure to risk of falls from within the excavation.

Bund Design and Construction

  • Bunds must be designed and constructed in accordance with:
    Haul Roads:
    • GRM PLN Road Design Specification for the Design and Construction of Haul Roads
    Dumps / Tipheads:
    • BMA GDL Geotechnical Truck Dump Design and Operating Guideline
    Hazardous Areas:
    • 100.01 GRM SOP Workplace Inspections and Control to Hazardous Area

1a) Haul Road Bunds

  • Installed when -
    • the roadside drop-off is greater than 0.5m;
    • the roadside batter angle is greater than 1:4 (vertical: horizontal);
    • there is an unacceptable risk of collision between vehicles and roadside obstacles.

  • Constructed according to -
    High risk areas (in the direction of a roadside drop off with a batter greater than 3V:1H) .
    • A minimum of 3 metres high
    • Batter slope of 1V:1H
    • Minimum base width of 7 metres

  • Non high risk -
    • Half the wheel height of the largest vehicle in that area
    • Maximum of 1.8 metres high

  • *Note - All classes of safety berms must be lowered to 1m-1.2m through intersections for a minimum of 10m so that sight distance for light vehicles is not compromised.

  • Select 3 haul road locations and verify the following:

  • Are bund heights constructed to a minimum of half wheel height of the largest vehicle that accesses the area (ie. accounting for visibility at intersections or 3m for high risk areas)?

  • Review 3 Corvus Shift Logs:

  • Is there evidence of bund inspections being completed every shift?

  • Review 3 Geocortext Reports:

  • Is there evidence of bund compliance?

  • Was there documented evidence of non-conformances being immediately managed and rectified?

  • Select 2 personnel:

  • Do personnel have an understanding of haul road bund requirements (ie. when, height, width)?

1b) Dumps / Tiphead Bunds

  • Constructed to:
    Half wheel height of the largest truck operating the dump.

  • Select 3 in-pit dump / tip head locations and verify the following:

  • Are bund heights a minimum of half wheel height of the largest vehicle that accesses the area?

  • Review 3 Corvus Shift Logs:

  • Is there evidence of bund inspections being completed every shift?

  • Review 3 Geocortext Reports:

  • Is there evidence of bund compliance?

  • Was there documented evidence of non-conformances being immediately managed and rectified?

Standoff Distances

Primary and specified standoff distances are designed in accordance with 'BMA PRO Geotechnical Working Near Crests and Slopes'.

2a) Primary Standoffs (height aspects only)

  • Primary standoff distances are applied to all slopes steeper than 45o and relate to any person required to work within 5 metres of the crest.

2b) Specified Standoffs (height aspects only)

  • Vehicle access roads and park up areas above Tertiary and extremely weathered Permian slopes are not to be within 15m of the crest unless the JSA indicates that it is safe to do so.

  • Persons required to work within these primary and specified standoff distances must be either:
    i Trained in Ground Control Awareness and completed a risk assessment.
    ii Accompanied by a person trained in Ground Control Awareness and completed a risk assessment
    iii Working in an area covered by a JSA or a documented Work Area Inspection (signed off by a person trained in Ground Control Awareness).

  • *Note - Primary and Specified standoff distances are a minimum starting point, and may increase in geotechnical hazard areas.

  • Select 2 active work fronts and verify the following:

  • Are Primary and specified standoff distances are being applied?

  • Knowledge test 2 personnel:

  • Do personnel have an understanding of primary / specified standoff and bund height requirements?<br>

  • Select 1 area that fits within the definition of tertiary or extremely weathered Permian slope:

  • Have the standoff requirements been met?

  • For persons that meet the requirements for working within primary and specified standoffs:

  • Is there a risk assessment that appropriately identifies / manages associated hazards and the controls have been applied?

  • Have personnel in the work group been trained and recall the Ground Control Awareness requirements, and requirements for standing on bunds (ie. never stand on-top of bund)?
