Risk Scope:
Fall from Height includes single person scenarios such as fall from work platforms, mobile scaffolds, removable handrailing or ladders with or without restraint / arrest systems .
Scope excludes Material Risk elements, such as Man Boxes, EWP's, Sully Boxes, clip and tube scaffolding etc. -
Control Objective:
Non Fixed Work platforms, removable handrailing or ladders are designed and maintained to prevent failure of critical components during use and ensure the safety of personnel.
Non Fixed Work Platforms, Removable Handrailing or Ladders
Non Fixed Work platforms, removable handrailing or ladders used on site are designed, installed, maintained and operated utilised in accordance with AS/NZS 1657 - Fixed Platforms, Walkways, Stairways and Ladders & AS/NZS 1892 Portable Ladders - Design, Construction and Installation, OEM and legislative requirements.

1a) Ladders (both portal and fixed) are designed, constructed, maintained and used in accordance with the 'GRM STD Scaffolding and Ladders Standard'.

Select 4 ladders (2 portable, 2 fixed) and verify the following:
-
Are there slip resistant surfaces/ treads on the walking surfaces or non slip tread on the nose capping?

If BMA owned, is the ladder identified with a unique number, and managed through the SAP system?

If Contractor owned, is the ladder tagged with a unique number and next inspection date, and entered into the contractors ladder register?
-
Is the ladder maintained to a good condition? (Check for any modifications, damage or defects).

1b) Non fixed work platforms, and removable handrailing used at sites are designed, constructed and maintained in good fit for purpose condition

Select 2 Coal Mine Workers (CMWs) involved in the use of portable and fixed ladders, and verify the following:

Has the correct type of ladder been selected for the task?
-
Is the ladder the correct length for the task, ands do the workers know the maximum permitted ladder length requirements ( i.e. 6.1m for single ladder, 9.2m for extension ladder used to do electrical work, 7.5m for all other extension ladders )?

Is the ladder secured to prevent unintended movement?
-
Is the ladder being used as intended (i.e. 3 points of contact maintained, correct height / base ratio)?

Non fixed work platforms and removable handrailing used at sites are designed, constructed and maintained in good fit for purpose condition

1c) Check 2 non fixed work platforms and verify the following:

Has the non fixed work platform got a SWL?

If the platform is fabricated, has it got a plaque attached with the drawing number and date of fabrication?

Is the non fixed platform maintained to a good condition? (Check for damage or defects).

Is the work platform being used as intended ( i.e. correct access point being used, workers remaining within handrails, and gate / handrails secured )?

Check 2 removable handrails and verify the following:

Do all Installed handrails have a height of no less than 900mm or greater than 1100mm at all work platform levels?

Is the minimum distance between top and mid rails greater than 550mm?

Is the removable handrail maintained to a good condition? ( Check for damage or defects).

1d) Select 2 Coal Mine Workers (CMWs) involved in the use of removable handrails (e.g. chassis), and verify the following:

Are the handrails installed correctly?
-
Are the handrails seated and secured correctly?
-
Does the CMW maintain three points of contact when accessing and exiting from the handrails area?
-
Is the work area kept free from slip and trip hazards?
