  • Risk Scope:

    Scope includes all vehicles, mobile equipment and other equipment fires e.g. dragline house fires, shovels, light, medium and heavy vehicles. High risk scenarios include personnel unable to escape from equipment, disorientation / inhallation of smoke. Scope excludes bushfires, tyre fire and explosives (blasting).

  • Control Objective:
    To ensure coal mine workers are not exposed to an uncontrolled fire/explosion event through fire detection and preparedness requirements are resourced and maintained to alert and reduce the impact from uncontrolled fire.

Fire Systems for Vehicles

Mobile equipment design, maintenance and operation must include fire systems to detect, suppress, fight and alert personnel of potential uncontrolled outbreak of fire.

  • A system must be in place to detect and
    reduce the impact from potential and equipment fires, such as reasonable access to fire fighting equipment and suppression systems where applicable.

  • 1a) Select 2 items of mobile equipment and verify the following:

  • Fire fighting equipment and suppression systems is readily accessible and access ways are clear?

  • Are onboard fire suppression system is in date, pressurised and statutory inspection last 6 months?

  • Are all onboard portable fire extinguishers are in date, pressurised and statutory inspection last 6 months?

  • 1b) Select 2 items of mobile equipment and verify the following fire system records:

  • Are statutory testing on fire detection and dousing systems in date?

  • Are corresponding 1SAP work orders in place for routine maintenance of fire detection and fire fighting systems (i.e. 6 monthly inspection, 12 monthly practical test)?

  • Does the fire detection inspection report (Chubb) reflects the equipment and up to date?

Plant, Building and Equipment Design

Mobile equipment is designed in accordance with legislative, code of practice and BMA Engineering Standards for emergency egress.

  • Mobile equipment is constructed and operated in a manner which ensures two forms of emergency egress.

  • 2a) Select 2 items of mobile equipment and verify the following:

  • Is there two forms of adequate emergency egress (check for clear access way, integrity, signage etc).?

  • 2b) Select 2 mobile equipment operators and verify the following:

  • Do personnel understand the emergency process?

  • Can the person identify and locate two forms of emergency egress for the mobile equipment?

Emergency Response

Emergency response is conducted in accordance with legislative requirements, Australian Standards and BMA specifications.

  • Emergency process is carried out in accordance with the 'GRM PLN Emergency Response Plan'.

  • 3a) Select 2 mobile equipment operators and verify the following:

  • Are emergency response responsibilities understood and is the process clear?

  • Do they know how to raise an emergency?

  • How would they respond when an emergency has been raised?

  • In the event of a fire or suspected fire onboard the mobile equipment, how would they respond (i.e. raise emergency, make equipment safe, set off fire suppression manually if automatic actuation has not occurred, exit from the equipment, fight fire if feel safe to do so, ( NOT tyre fire ), retreat from fire to safe position if not feel safe to fight the fire and wait for assistance)?

  • Can the operator explain how to manually activate the fire suppression system?

  • Can the operator explain how to remove a portable fire extinguisher from the mounting bracket, how to set the portable fire extinguisher off and how they would use the portable fire extinguisher when fighting a fire?

  • In the event of a fire or suspected fire onboard rubber-tyred mobile equipment, how would they respond? (i.e. activate the emergency, park equipment up safely (where possible, not within 300m of any populated area), park with sidewall(s) of affected tyre(s) facing an earth wall or bunding if possible, shut engine down and activate fire suppression, do not attempt to fight tyre fire with a hand held extinguisher, exit equipment from opposite side of the fire walking away from the front of the equipment - do not walk to the side of the equipment or inspect tyres, exclusion zone of 300m established and area restricted for 24 hours)
