  • Risk Scope:
    High pressure release from pipes, lines and storage vessels resulting in burns, injection or impact. Injection includes any liquid or gas injected into the body at high pressure. May include the release of grease, air, hydraulics, nitrogen, and water, in addition to identifying stored energy hazards prior to transportation of pressure vessels / systems off-site. Scope excludes tyre pressure release (covered in tyre or rim incident).

  • Control Objective:
    To ensure workers are not exposed to incidents involving the uncontrolled elease of high pressure energy through the effective de-energisation and isolation of pressurised equipment

Pressure Vessels and Pressure Systems

Pressure Vessels and pressure systems must be designed, installed, maintained, operated and utilised in accordance with relevant Australian Standards, manufacturers instructions, legislative requirements and site standards.

  • 1a) Pressure Vessels must include a system to release stored energy, such as safety release valves, bleed taps, etc.

  • Select 2 maintenance strategies and audit / validate in the field for the following:

  • Is the release valves of 1 insitu pressure vessel safe?

  • Are isolation points adequate?

  • Is wear and tear on supply lines and fittings acceptable?

  • Are gauges legible and in good working order? (If relevant, obtain optimal pressure range and assess if gauge is reading in a safe operating range).

  • Are equipment free from any visible leaks?

  • 1b) A system must be in place to ensure pressure vessels sent off site for modification or repair, are at zero energy condition (as close as reasonably possible) prior to transport off site.
    • Where zero energy condition is not possible an Information Tag must be fitted to the vessel, informing others of the stored energy remaining in the vessel.

  • Select 2 different types of equipment intended for transport (eg. brake accummulator, fire extinguisher), which have the potential to contain stored energy and verify the following:

  • Is stored energy tag placed in a prominent location and populated clearly?

  • Has zero energy condition been achieved / verified (e.g. plugs/valves are removed ensuring an open circuit)?

  • Are information tags in place where zero energy is not possible to verify or achieve?

  • Select 2 personnel (eg. 1 Maintenance & 1 Warehouse).

  • Do personnel understand how to identify, handle and/or dispose of pressure vessels?

  • Has hydraulic equipment been tested by a 3rd Party?

  • 1c) Identify all energy sources (i.e. incoming and stored energy), and equipment / contents with the potential to release high pressure energy.
    Apply isolation and test for zero energy prior to commencing work.

  • Select 2 Coal Mine Workers (CMW's) performing work which involves pressure vessels and/or pressure systems.

  • Do personnel understand associated hazards, and management strategies to prevent high pressure energy release / exposure?

  • Does the risk assessment for the task adequately identify potential hazards, control measures and are controls implemented?<br>

  • Obtain 2 Isolation Confirmation Certificates (ICC) where applicable for works being undertaken on a pressurised system and verify:

  • Are all upstream/downstream isolation points positively isolated and the system de-energised?

Design and Management of High Pressure Injection

The design and management of High Pressure Injection (HPI) is carried out in accordance with legislation, Australian Standards and internal BMA requirements including the 'GRM PRO High Pressure Injection Procedure'.

  • 2a) For any CMW who receives a confirmed or suspected fluid/gas injected under the skin, the following must be carried out:
    • Initiate emergency, area made safe, notify Supervisor and CMW transported immediately to Medical Centre;
    • First aid treatment by Paramedic (including gentle cleaning of injured part, immobilisation and elevation of the affected limb);
    • Copy of SDS obtained for involved substance;
    • CMW not to intake fluids or food (ie. to prepare for potential surgery).
    • CMW transported to hospital (ie. with Treating Practioner Letter, SDS).

  • Select test 2 personnel (1 Paramedic and 1 Health Advisor) and verify the following:

  • Are the requirements for managing a known or suspected high pressure injection injury understood? (eg. "Can you explain the process should a CMW receive a high pressure injection injury").

  • Do personnel know where to obtain a copy of the procedure (ie. iPick), SDS's and in the case of the Paramedic the 'Treating Medical Practioner Letter'?
