  • Risk Scope:
    Includes all offsite travel e.g. travelling for work purposes and bus travel to and from Moranbah and GRM for major shutdowns. The inclusion of daily travel to and from work is limited to to hours of work provisions. GRM maintains roadways to Moranbah in conjunction with Moranbah North. Note - Off-site Vehicle Incident (Within the Hours of Work) is not considered a 'Category 1 - Controlled' risk. Out of scope scenarios include maintenance of public roads, verification of driving behaviours and other third party interactions on the public roadway.

  • Control Objective:
    Ensure the safety drivers travelling to and from work and other locations for work purposes (within the Company's span of control).

Fatigue

Fatigue requirements are designed, maintained and operated in accordance with legislative, Australian Standards and internal BMA / GRM requirements including 'GRM PRO Hours of Work Procedure'.

  • Hours of work are conducted in accordance with the 'GRM PRO Hours of Work Procedure', unless a risk assessment is completed, signed by the Site Senior Executive (SSE). Requirements include:
    .• The maximum Shift Length must be 14 hours.
    .• The maximum Gross Shift Length must be 15 hours.
    • Maximum working time (and gross working time) in a week is not exceeded for the roster type.
    .• Under no circumstances must Shift Length be extended beyond 16 hours.

  • Select test 3 personnel (including at least 1 Supervisor) to verify the following:

  • Do personnel understand the permitted hours of work?

  • Obtain a copy of the Fatigue Risk Assessment for the major bus contract, and verify the following:

  • Has the fatigue risk assessment has been completed in full, and signed off by the appropriate Manager and/or Site Senior Executive?

  • Select 2 bus drivers and verify the following:

  • Is there compliance to the control requirements? (i.e. interview / check SureSite records)

Roadway Designed and Maintainance

Roadway designed and maintained in accordance with GRM maintains roadways to Moranbah in conjunction with Moranbah North

  • Non-public road aspect maintained by Goonyella Riverside Mine between Moranbah and GRM is adequately maintained.

  • Physically drive the aspect of the road-way that is maintained by GRM:

  • Does the roadway have any deficiencies?

  • List the deficiencies:


  • Select 1 Coal Mine Worker (CMW) from the Maintenance Infrastructure Department (i.e. involved in co-ordination of road maintenance) and verify the following:

  • Have deficiencies identified as part of the initial observation been identified and entered into SAP?

  • Is an inspection process in place to identify and rectify non-conformances?

  • Select 3 inspection reports from the last 6 month period, and verify:

  • Have deficiencies been raised as sub-notifications and completed?

Journey Management Plans

Journey Management Plans are designed and operated in accordance with BMA requirements, as specified in 'GRM STD Vehicles and Mobile Equipment Standard - Journey Management Plan'.

  • A formal Journey Management Plan must be developed for the following circumstances:
    a) Drivers are expected to drive more than six hours in one day;
    b) Drivers are expected to drive on a route that is unfamiliar or the driver has not travelled it in the past 12 months;
    c) Drivers Fatigue Risk Profile (determined using the BMA Fatigue Calculator) is calculated at Medium or High.

  • Select 3 cases of workplace travel within the last 6 months that meet the threshold of the journey management criteria, and verify the following:

  • Was the travel approved, journey management plan carried out / documented, and identified controls implemented? (i.e. interview individual)


  • Select 3 personnel from different Departments:

  • Do personnel understand the application of the Journey Management Plan and it's requirements?
