ACCESS & LIGHTING

  • Work areas are free of shadows and barriers blocking light.

  • There is enough light for visibility without straining.

  • Dangerous areas/equipment are well lit to alert employees of hazards.

  • Adjustments are in place where needed for persons with visual limitations.

  • Printage and signage materials are of adequate size/color/contrast/sheen.

  • Lighting units are on, clean, and in working order.

BURNS

  • Heat generating equipment is insulated.

  • Hazardous materials are stored in suitable conditions.

  • Operators are not at risk of chemical exposure due to splashing materials.

  • Wiring insulation is in good condition.

  • Warning signs are posted where burn hazards cannot be otherwise mitigates.

CUTS AND STRAINS

  • Equipment is free of rough edges.

  • Used sharp equipment is properly disposed.

  • Mechanical assistance is available and adequate.

  • Adequate fluids are available for employees to hydrate.

  • Employees are not exposed to extreme heat.

ELECTRICAL WIRING

  • All lights switches have covers.

  • All receptacles have covers.

  • No blanks are missing from electrical boxes.

  • Extension cords are not being used in lieu of permanent wiring.

  • There are designated LOTO for all electrical equipment in the area.

  • All unused electrical equipment is marked properly.

  • Electrical cabinets are shut properly and not left open.

  • Electrical cords are in good condition.

  • There is proper stress relief on flexible cords.

  • Flexible cords in use are the proper size for the load they are carrying.

  • All electrical equipment zones are marked and free of debris.

  • All cabinets have the proper NFPA labeling.

EQUIPMENT GUARDING

  • Appropriate machine guarding is in place.

  • All machine guarding is secured and cannot be easily removed.

  • There is no evidence of guards being by-passed.

  • There is no access to moving parts through any openings in the guard.

  • Guards are in good condition.

  • Electrical cords are in good condition.

  • There are no broken pieces or parts.

FALLING OBJECTS

  • Stacks of pallets and materials are straight and below the specified heights.

  • Tools and equipment are secured and in good condition.

  • Adequate barriers are in place to protect employees at ground level.

  • Gaps in handrails and steps meet OSHA requirements.

  • Warning signs for overhead work/falling objects are present and in use.

  • Stored objects do not represent a falling hazard.

FALLS

  • Equipment has adequate fall prevention equipment as needed.

  • Fall prevention equipment is adequately maintained.

  • Ladders are secure, inspected and suitable for tasks.

  • Stairs and railings are secure and adequate for work/tasks.

  • Stair treads are the appropriate height.

  • Lift equipment is stored properly.

  • Lift equipment is available and appropriate for routine work tasks.

  • Pre-use inspection checklists for lift equipment are completed as needed.

  • Platforms are adequately installed and maintained.

  • Tie off points for fall protection are identified and marked.

  • Fall protection equipment meets OSHA standard.

  • Fall protection equipment usage is inspected and maintained.

HEAD BUMP

  • There are no protruding objects or equipment.

  • Work spaces are not tight/cramped.

  • Working areas have adequate head room.

  • Work areas are free of head bump hazards.

IMPROPER STORAGE

  • Storage equipment is marked for specific use.

  • Storage equipment is used as intended.

  • Equipment load ratings are marked (if applicable).

  • Storage shelves over two shelves high are anchored.

  • Heavy materials are stored on shelves below chest height.

  • There are no pinch points in the storage areas.

  • Storage areas are clean and free of debris.

  • Storage areas do not block exits, fire extinguishers, etc.

  • Aisles and passages have adequate clearance.

  • Storage areas are designed and utilized to promote safe lifting techniques.

PERSONAL PROTECTIVE EQUIPMENT

  • Hearing protection is readily available to employees.

  • Hearing protection is being worn (fully inserted) as required.

  • N95 dust mask respirators are stocked at PPE stations.

  • N95 dust mask respirators are stored correctly.

  • Face shields are readily available to employees.

  • Organic vapor cartridges are available within the work area.

SLIPS & TRIPS

  • Surface is level and free of cracks, holes, uneven spots, and obstructions.

  • Ramp surfaces are even, well lit, and free of clutter/materials.

  • Ramp surfaces grip in all conditions.

  • Pathways are free of water or other liquids.

  • Pathways and surfaces are free of obstacles.

  • Housekeeping is adequate.

  • There are no visible leaks or spills.

  • There is adequate signage to inform employees of hazard.

  • Equipment (hoses, cords, tools) is not routinely left on surface as part of work procedures.

VEHICLES & POWERED EQUIPMENT

  • High traffic areas are marked and clear of obstacles.

  • Signs are present as needed to warn pedestrians.

  • Signs are present as needed to caution operators.

  • Gates between high traffic and pedestrian walkways are present and in good condition.

  • Pre-use inspection checklists for powered equipment are completed as needed.
