Does the operative have a scope of works on site and are they aware of required works?

Have hazards been identified and suitable risk assessments selected and recorded using BKH F053 Task Specific Risk Assessment Register?

Do technicians carry the full Task Specific Risk Assessment collection?

Are operatives fully aware of the risks / hazards associated with the operation(s); have the customer / visitors been informed of any hazard that could affect them?

Is a method Statement required on site and if so has it been completed using BKH F090 Method Statement detailing full operations and associated control measures?

Are all required COSHH assessments for hazardous substances on site?<br>If no COSHH assessments required enter N/A

Do technicians carry the full chemical folder including MSDS, COSHH and Usage instructions?

Are there any manual handling procedural issues?<br>If no manual handling please state N/A

Is appropriate PPE available and being used?

Are all operatives fit for work and not under the influence of alcohol or drugs?

Have any operatives had any accidents whilst on site?

If an accident has occurred, has it been reported according to procedure?<br>

Are first aid provisions available?

Has an ACM survey been completed?

If there are working at height operations taking place are correct procedures in place with a suitable TSRA and is the equipment suitable?

If working on electric have services been isolated by qualified / competent persons?

If there is any mechanical tools / plant being used on site is it all PAT tested or within thorough inspection regime and in good working order?

If there are any areas of work deemed to be a confined space are the correct procedures in place with suitable RAMS?

If the operative is lone working are correct procedures in place with suitable TSRA?

Is adequate housekeeping maintained?

Is there any waste produced and have any relevant environmental issues been complied with?

Is the operative supervised / monitored?

Has a serious non-conformance been identified which has resulted in the works being suspended i.e. Work stopped whilst operative was asked to complete a TSRA?
