S1 OH&S Policy

  • 1. Has the Policy been communicated to all persons working under the control of the organisation with the intent that they are made aware of their individual OH&S obligations?

S2 Planning

S6 Communication Participation and Consultation

  • 1. Do you have a procedure covering communication with contractors and visitors on site?

  • 2. Do you have a procedure for receiving, documenting, responding and recording relevant communications from external interested parties?

  • 3. Are workers, where appropriate, involved in hazard identification, risk assessment and the determination of controls?

  • 4. Are workers, where appropriate, involved in incident investigations, the development and review of OH&S policies and objectives, consultation where there have been changes that affect their OH&S and representation on OH&S matters?

  • 5. Are workers informed about their participation results and who their H&S representative is?

  • 6. Are contractors consulted when there are changes that could affect their health and safety?

S7 Control of Documents

  • 1. Is there a system in place to ensure that only relevant versions of applicable documents are available at point of use?

  • 2. What controls are in place to prevent obsolete documents being used?

  • 3. Are documents regularly checked to ensure that they remain legible and readily identifiable?

8. Emergency Preparedness and Response

  • 1. Have the emergency services been advised of the hazards on site and the procedure relating to your emergency plan?

  • 2. Have neighbours (if applicable) beed advised of the procedures relating to your emergency response?

  • 3. Have tests of your emergency procedures been carried out and where the emergency services involved?

  • 4. Have you reviewed your emergency procedure following this exercise?

9. Performance Measurement and Monitoring

  • 1. Has monitoring of workplace hazards and employees health been completed within the last year ( this may include dust, noise, medicals)?

  • 2. Has a review of the sites objectives and targets been completed and the findings recorded?

  • 3. Has the effectiveness of any control measures been established (LEV systems, lifting equipment, guarding, PPE)?

  • 4. Are workplace inspections/observations carried out to establish conformance with safe working procedures or any other control measures required?

10. Incident Investigation

  • 1. Has the site identified the need for corrective action or opportunities for the preventative actions following an accident/incident?

  • 2. Have you identified opportunities for continual improvement?

  • 3. Have the results of any accident investigation been effectively communicated?

  • 4. Are the results of incident/accident investigations documented and maintained?

11. Nonconformity, Corrective and Preventative Action

  • 1. Have any non conformities been investigated and suitable actions taken to mitigate their OH&S consequences and prevent recurrence?

  • 2. Have the actions been recorded and communicated?

  • 3. Have the proposed actions been subject of a risk assessment prior to implementation?

12. Management Review

  • 1. Has a management review been completed for the site within the last 12 months?

  • 2. Have the results of the review been communicated, discussed and acted upon as required?
