Equipment Fire Drill Checklist =Please list of all staff who participated in the drill, and any visitors participating.

  • Guidelines: During the Drill - Designated area, complete the checklist below. Mark each item with a Pass/Fail or Yes/No - check N/A if the item does not apply. Note: Document corrections in comment section.

Equipment Fire Drill Log

Check all steps operator completes correctly. Make a comment on steps operator needs retraining on.

  • 1. Chassis or Container placed on ground

  • 2. Supervisor clearly notified on radio of situation

  • 3. Unit pulls away from chassis or container

  • 4. Unit placed in park, brake applied (lower/or move cab to exit position if needed) and engine turned off

  • 5. Operator explains how to activate fire suppression system (if unit equipped) without activating.

  • 6. Operator safely exits unit using 3 points of contact when possible

  • 7. Master switch turned off

  • 8. Operator explains how to use a hand held extinguisher and what hazardous situations (large fire or high heat) would prevent using a hand held extinguisher.

  • 9. Operator explains how to open engine compartment and what hazardous situations (large fire or high heat) would prevent this activity.
