  • If you have safety concerns, are the adequately addressed during the tailboard, prior to beginning the job?

  • Do you feel you have adequate training for your job?

  • Are distractions identified and addressed prior to the beginning of the job?

PHYSICAL CONDITIONS

PERSONAL PROTECTIVE EQUIPMENT (PPE)

  • Are all employees wearing the appropriate PPE for the task being carried out?

  • Is the PPE (that was observed) well maintained?

  • Are the Rubber Gloves valid? (check date)

EMERGENCY PREPAREDNESS

  • Does the site have an emergency evacuation plan?

  • Are emergency phones working?

  • Do the emergency phones have a current emergency phone list posted?

LIGHTING

  • Does the emergency lighting work?

  • Is the emergency lighting tested regularly?

  • Is the general lighting adequate?

FIRE CHECKS

  • Are all fire-fighting equipment checked on a regular basis?

  • Are fire doors closed and not blocked open?

  • Are flammable cabinets grounded?

  • Is there clear access to fire equipment?

PRE-USE INSPECTION

COMPRESSED GAS CYLINDERS

  • Are compressed gas cylinders properly secured?

  • Are compressed gas cylinders stored properly?

SIGNAGE / LABELS

  • Is the proper signage posted? (noise, radiation, confined space, asbestos)

  • Are containers properly labeled?

MISCELLANEOUS

  • Are portable ladders in good condition?<br>(feet, rungs, blooming (spider web), etc.)

  • Are the portable ladders in use, secured?

  • Are fixed ladders in good condition?<br>(rungs, safety chains, rust, anchored securely, etc.)
