  • This audit is to be used when conducting a spot inspection on a Joy Global passenger truck/ SUV. As of June 2014 this audit does not replace the Vehicle Safety Inspection Audit that Is to be performed by the operator on a daily basis and kept in the vehicle at all times.

EXTERIOR

  • Is the overall condition of the exterior decent and free of large dents, missing parts, broken parts, etc.

  • Check all the following lights: Brake lights , head lights, running lights, tail lights, emergency flashers and turn signals. Are they all working correctly?

  • Are the windshield wipers blades in good condition (showing no signs of being ripped or dry rotted) and operational?

  • Are the tires in good condition with proper tread and free of damage?

  • Is the windshield, all windows and side mirrors free of cracks, excessive rock chips and excessive dirt build up?

  • Trucks only- is the bed clean with no loose debri and all items secure? (SUV select N/A)

UNDER THE HOOD

  • Is the engine oil at the proper level?

  • Is the power steering fluid at the proper level?

  • Is the coolant at the proper level?

  • Is the transmission oil at the proper level? (If inspected a Nissan mark N/A - this does not apply to the Nissan trucks or SUV's- They have a sealed transmission and is only accessible thru the dealership or other maintenance shop)

  • Is there windshield washer fluid present?

INTERIOR

  • Is the general interior of the vehicle maintained with minimum loose objects and clutter?

  • Is the fabric on the seats and the roof liner in good condition with little or no tears and rips?

  • Are the brakes operational to include the emergency brake?

  • Is the steering good?

  • Are all seat belts present and operate correctly according to manufacture?

  • Is the rear view mirror present and in proper order?

EQUIPMENT

  • Is the fire extinguisher present with the following: at least 5lbs, easily accessible, secured, up to date annual inspection, up to date monthly inspection, labels/stickers intact and readable, gauges in green, hose in good shape, pin secured in place?

  • Is the buggy whip present, in good condition, can extend up to 14ft and have an operational light?

  • If equipped, Is the back -up alarm operational

  • Is the horn operational?

  • Is the first aid kit present, stocked and up to date with no expired items?

  • Are the chock blocks present with no cracks, free of excessive damage, and are not of hollow type?

POLICY/CUSTOMER/MSHA Requirements

  • Is the daily vehicle inspection book present, up to date with that days pre-operation inspection completed and filled out correctly?

  • Was the parking break set?

  • If this vehicle is currently on any FCX/FMI site, is the current Contractors Safety Manual present with part one and part two? (mark N/A if not on a FCX/FMI property)

ADDITIONAL REMARKS AND COMMENTS

COMPLETE AND REVIEWED

  • Completed Audit/inspection needs to be sent to the Manager and Supervisor of Vehicle being inspected for corrective actions to be addressed and to the Health and Safety Department for record keeping.
