  • By signing this completion certificate to submit for final payment, the signing Affinity field supervisor and general contractor have personally reviewed and verified, onsite together, all labor and materials invoices to the Owner/Affinity Building Solutions, LLC for this job. Furthermore, the field supervisor and general contractor hereby represent all labor and materials charged to owner for this project are 100% accounted for and installed in accordance with the terms of the contract. In the event something is charged and found missing or uninstalled at a later date, the field supervisor and general contractor acknowledge their responsibility for such missing work to which the payment was authorized and charged. The signers to this statement understand their responsibility to ensure the accuracy of all completed work and respective charges paid on this job. Any false statements will be prosecuted in accordance of State Federal laws as applicable.
