Jobsite General

  • Is any form of site specific training/orientation being held?

  • Is the First-Aid kit available and fully stocked?

  • Send out fully stocked First-Aid kit

  • Is an eye wash station available and filled properly?

  • Onsite but Not Full?

  • Fill station properly and make accessible

  • Are safety signs posted and readable?

  • Furnish/replace signs to show proper safety precautions

  • Are emergency phone numbers posted?

  • Post numbers where visible to workers

  • Has adequate access to the site been provided?

  • Notify superintendent to make sure that site is accessible in a safe manner for all

  • Is all protruding rebar properly guarded?

  • Cap all exposed rebar ends with OSHA cap

Housekeeping

  • Are walkways and stairs clear and kept free of debris?

  • Clean areas to avoid slips, trips, and falls.

  • Is the temp. lighting adequate and is it in good working condition?

  • Is there enough of it?

  • Add more temp. lighting

  • Are there bulbs in every socket?

  • Put bulbs in every socket

  • Are all bulbs protected?

  • Protect each bulb with properly installed cages

Fire Protection and Prevention

  • Are fire extinguishers on site and serviced properly?

  • Provide fire extinguishers/switch out expired fire extinguishers

  • Are all flammable liquid containers properly identified?

  • Make sure all are clearly marked with "Flammable"

  • Are the acetylene and oxygen bottles stored correctly?

  • Are bottles secured upright?

  • Secure each bottle in upright position

  • Are the bottles capped and separated by at least 20 feet?

  • Cap and separate properly

Personal Protective Equipment (PPE)

  • Are all employees wearing hard hats, safety glasses, and high visibility clothing on the outermost layer?

Fall Protection

  • Are ladders properly setup and being used correctly? (tied off, 4:1 angle, extend 3' above)

  • Get the correct ladder for the job being performed

  • 4:1 angle?

  • Correct to required 4:1 angle

  • Step ladder fully opened?

  • Open Fully

  • Extended 3' above?

  • Extend ladders 3' above

  • Is ladder tied off if necessary?

  • Tie off properly

  • Are employees standing on the top 2 steps of the step ladder?

  • Get off the top steps and find a taller ladder for the job

  • Is the ladder damaged or defective?

  • Take out of service immediately

  • Are employees using proper fall protection methods?

  • Are employees wearing the fall protection properly?

  • Are the perimeter cables/guard rails properly installed?

  • Are cables/guard rails installed at all required locations?

  • Install perimeter cables/guard rails properly at all required locations

  • Are cables/guard rails installed at proper height?

  • Make sure all cables/guard rails are between 39 and 45 inches in height

  • Is there both a top and mid rail in place?

  • Make sure all top and mid rails are in place and secure

  • Are toe boards required/installed correctly?

  • Install toe boards correctly

  • Is the scaffolding setup correctly?

  • Height/Width ratio aceptable

  • X-Braces in place

  • Install X-Braces properly

  • Guard rails in place/tied off

  • Install guard rails properly or tie off

  • Pins in place

  • Put all pins in proper locations

  • Fully planked

  • Plank fully with proper planks

  • Wheels/Base Plates in place

  • Install wheels/base plates

  • Height:Width ratio acceptable

  • Tie into building or add outriggers

  • Has the scaffold been inspected daily by a competent person?

  • Inspect daily and use scaffold tags

  • Are floor holes properly covered and protected?

  • Cover, Protect, and Label Exposed Floor Holes

Electrical

  • Are all extension cords in good working condition?

  • Took out of service

  • Is the electricity on site being used through a GFCI or welder?

  • Make sure all cords are run through a GFCI

  • Are the electrical panels labeled, locked, and free of holes?

  • Labeled

  • Label panel correctly

  • Locked

  • Lock panel

  • Cover all holes

  • Are all hot outlets and switches properly covered?

  • Put covers on or cut power

Trenching

  • Are the trenches properly protected against cave in?

  • Is there a competent person on site for daily trench protection?

  • Shut down trenching operations until there is one

Tools

  • Are guards in place and used correctly on all tools that require them?

  • Stop using tools immediately and replace all guards

Mobile Equipment

  • Is the mobile equipment on site in good working condition?

  • Repair/Replace Equipment Immediately

  • Are crane barricades properly installed and rigging in good condition?

  • Barricades installed

  • Install crane baricades

  • Rigging in good condition

  • Replace with proper rigging

  • Is the 10' minimum distance from power lines maintained?

  • Adjust equipment accordingly
