JOB SITE CONDITIONS

  • 1.There is a Safe Access going in and out of the machine Room.

  • 2. Machine room slab with holes according to details

  • 3. Safe access to the Controller Area (MRL)

  • 4. Safe Working Area in front of the Controller (MRL)

  • 5. Temporary Main Power Supply Available (GFCI, Main Switch and Grounding)

  • 6. Temporary lighting power supply available (GFCI, Main switch and Grounding)

  • 7. Safety signages are installed at conspicuous areas such as electrical supply, landing door openings, machine rooms.

HOISTWAY & OVERHEAD PROTECTION

  • 1. Dimensions and overhead according to the lay out

  • 2. Standard car weights are highlighted based on duty and elevator model and posted at the bulletin board. (Included on the Partnering handbook)

  • 3. Full Barricading are in place – (Landing openings, Machine room block out holes, fixture openings) are netted & covered.

  • 4. Full Barricading are installed with M-12 expansion bolts. (Provide photo of each floor level.)

  • 5. Certification has been provided for all lifting points installed by the General Contractor, signed by the structural engineer and posted at the bulletin board. (Specified load indicated on the certificate)

  • 6. Each lifting point is tagged with its safe working load signed by project engineer and with complete details

  • 7. No other Trades working in the hoist way

  • 8. Hoist way is clear of a non-elevator Equipment

  • 9. Safe access to the landings (No fall hazards, Adequately Lit)

  • 10. Pit depth according to the Lay out.

METHOD AND MANUAL

  • 1. Field Installation Manual available on the jobsite in local language/ language of those who will install the unit.

  • 2. Construction Safety and Health Program available at site.

FALL PROTECTION AND ELEVATED WORKS

  • Personnel Protective Equipment Checklist Form has been completed and all employees have a complete set of PPE visually inspected at the work site.

  • Fall Protection Equipment has been inspected and tagged with inspection sticker for every lanyard and body harness, there is one set of fall protection per worker

  • 2 Lifelines per shaft are securely installed and conform to Otis standard (red strip woven into the rope).

  • Scaffolding equipment is fully planked with overhead netting installed OR Cantilever platform has been inspected and is safe to use.

  • For all running platforms, the safety gear and governor has been tested or skylock has been tested.

  • There is 1 functioning rope grab per lifeline on site. Lifeline puller available.

TOOLING & EQUIPMENT

  • Tooling & equipment required by the Field Installation Manual are available

  • The false car is equipped with safety signages and third-party certificate from DOLE accredited testing organization is posted at the jobsite. (Valid for 1 year). If new equipment, provides proof of date of purchase (receipt, invoice etc.)

  • The false car pre-operational safety checks form has been completed and blank copies are available at the false car to carry out the daily checks. Wire rope extends from machine room lifting point to pit floor.

  • All electrical portable tooling & equipment have GFCI - 10mA for tools and 30 mA for welding and astro motor for false cars. GFCIs are inspected and tagged (Test and Verify if working).

  • 1 pc multi-meter is available test and verify if working.

  • Flash lights and whistles available for each worker.

  • LOTO kit is available at the work site, including at least 2 pcs padlock each with 1 key, 1 pc multi hasp, 2 pcs tags.

  • Subcons have sufficient mechanically protected (finger proof) lights installed at the shaft and connected to a 10mA GFCI ready test and verify.

  • Extension Wires are available and is made of royal cord.

WORKERS QUALIFICATION

  • Workers assigned are technically competent and Trained on Safety

  • Workers assigned have received the technical training for the installation methods as required by the FIM

  • Inexperience Workers have appropriate supervision

OTHER SAFETY REQUIREMENTS

  • Emergency evacuation route defined, and muster point identified.

  • JHA booklet, Partnering Handbook and Subcontractor Logbook are available on the site. (One for each shaft.)

  • Latest Learn and Live Bulletin is posted at the bulletin board.

  • Basic First aid kit is available at the jobsite available for use

  • There is an available bulletin board provided at the jobsite.

  • Fire Extinguisher are available at the jobsite and readily accessible for anyone to use. (1 for each shaft. See issuance valid for 1 year.)

  • MSDS is available at jobsite and posted at the bulletin board

  • Copy of Antigen results (RAT for old contracts for each employee at site)
