Does the driver / personnel have the correct PPE.

Have the crew completed a TAKE 5

Is there a SWMS for this task.

Is the truck / trailer correctly positioned and level.

Are exclusion zones in place for loading / unloading.

Is the area clear from obstructions (people, plant, vehicles and other objects).

Are all items to be loaded / unloaded packaged appropriately.

Do the restraint devices appear to be in a suitable condition.

Does the load appear to be restrained appropriately.

Does the load appear to be stable prior to unloading

Are all items safe for unloading

Are the items to be unloaded free from obvious signs of damage.
