  • Did OOW informed guests not to smoke & clear the dive deck?

  • Did OOW order crew to clean up the dive deck?

  • Did OOW assign a crew to call emergency services/insurance?

  • Did the second tender assist pulling diver on the tender?

  • Did the tender driver assess breathing & responsiveness and notified the OOW?

  • Was all the equipment cleaned/stored/O2 refilled if under 1700 psi?

  • Was help called for directly when the victim was found?

  • Did the crew member who was first with the injured victim assess the situation?

  • Stop and assess the overall environment and ensure that it is safe to respond and administer first aid. Do not put yourself or others in danger in order to provide first aid to the victim. Stop and assess the condition of the victim.

  • Did this crew member ensure that the victim was removed from further harm? If possible
