Audit Requirements

  • Have the following requirements specified in JH-MPR-WHS-010 Excavation and Trench procedure been implemented?<br>• Hazards identified, controlled, assessed and added in the Workplace Safety Management Plan and Workplace Risk Assessment<br>

  • Has a Permit Controller been assigned to the Workplace and do they monitor the permit system?

  • Has a JH-FRM-WHS-010-01 Excavation and Trench permit been raised for the works being conducted and has it been authorized by the permit controller and workplace manager?

  • Has the Excavation and / or Trench been deemed a Confined Space and have JH-MPR-WHS-009 Confined Space Entry Procedure requirements been met?

  • Has a Dial Before You Dig Request form been raised and have all affected under and above ground services been identified?

  • Has a TRA been completed for the Excavation / Trench Activity and has it considered underground services (electrical / pressurised gas etc.), surrounding soils, structures, possible waters sources, mobile plant and other activities being undertaken in close proximity?

  • Name of employee(s) undertaking excavation / trench activity appears on the task risk assessment (TRA)?

  • Has their manager / supervisor communicated the TRA to them and do they understand the process?

  • Are the employees able to identify the applicable hazards associated with the excavation / trench works to be conducted?

  • Does the excavation / trench have a safe means of access / egress for fauna & employees?

  • Photograph of access and egress points

  • Has shoring/battering designed by a suitably qualified person in accordance with the procedure?

  • Is the excavation / trench air quality routinely sampled to ensure that it is safe, i.e. emissions from plant operating in close proximity?

  • Details of Testing Regime:

  • Have barriers, signage and fencing been put in place to safe guard against falls?
