Risk Management

  • Review the Workplace Risk Register to ascertain that Working on/over Water risks have been identified and listed?

  • Are WO/OW risks identified using a consultation process and are these risks covered in the Activity Method Statement?

  • The Workplace Manager is fully conversant with all risks identified in the WRA including how they are controlled?

  • Was the hierarchy of controls used to develop controls for the identified risks?

  • Has an attempt been made to eliminate risks?

  • Are workers carrying out a Task Risk Assessment on their work and including WO/OW risks prior to commencement of the work?<br>(sight examples in the workplace)

Personnel Competency

  • All workers are inducted and where required, qualified for the operation of marine vessels, hydrocarbon spill response, emergency rescue? (Check personnel files for copies of qualifications and training matrix)<br>

  • Other workers have a clear understanding of the particular requirements for Working on/over Water? (interview a selection of workers)<br>

  • The Workplace Manager has provided resources and time for training of personnel including emergency response crew practices?

  • The various qualifications and verifications of competency for working on/over water activities are recorded in personnel files and on the training matrix?

General

  • Is there a clear understanding of the application of Working at Heights controls for working at heights over water? <br>(Working at heights above water requires Work at Heights control measures with rescues planned from above not from the water)<br>

  • For each activity where work is carried out on/over water, is there evidence of a risk assessment process that initially focuses on eliminating the need to work on/over water? (ie. Were safe work platforms or scaffolding considered)<br>

  • Is a process in place for checking weather conditions for WO/OW?

  • Is there a contingency plan in place for deteriorating weather conditions?

  • Are all vessels appropriately registered in the State/Territory of operation? (Check State requirements – there should be a plant register in place)<br>

  • Do emergency response processes consider fire, man overboard, environmental spills, loss of towed load and adverse weather conditions?

  • Are Pilot Exemption Certificates in place for vessel operation in pilotage areas?

  • Are materials at the workfront;<br>• Stored in minimal quantities?<br>• Adequately bunded?<br>• Adequately secured?<br>

  • Is there an effective and well known communication process in place for WO/OW?

  • Are life rings appropriately positioned, inspected and night lights working?

  • Where working at heights controls are in use – are personnel trained and is the equipment tested and tagged and on a lifting equipment register?

  • Are Environmental Licenses required for this workplace when working on or over water?

  • If there are license conditions – are they being met or exceeded?

  • Are spill kits maintained in appropriate locations and inspected regularly?

  • Where abrasive blasting and coating processes are carried out on or over water – are these work areas encapsulated?

  • Is public safety assessed at the set up of the workplace and are these controls checked and maintained?

  • Is the rescue boat appropriately positioned, maintained and are rescue boat exercises carried out regularly?
