EVALUATION

  • Are all activities performed in compliance with an accepted JSEA that is still adequate for the current tasks & conditions?

  • Do all persons appear to understand the requirements of the JSEA & have signed off?

  • Are the following types of plant currently registered:

  • Mobile cranes with a safe working load of more then 10t?

  • Tower cranes?

  • Truck-mounted concrete placing units with booms?

  • Plant is safe if used properly - there is documented evidence of in-service inspections & servicing & maintenance by a competent person that is carried out at a frequency in accordance with the manufacturers' or suppliers' specifications or other approved standard. This will be identified with a site compliance sticker issued onsite.

  • Operators of the following types of plant hold a certificate to work in the prescribed occupation, but are not limited to: (Select all relevant to this observation)

  • Non-slewing mobile crane

  • Slewing mobile crane

  • Materials or personnel hoist

  • In all cases, was the plant operated by competent operators with documented evidence of skills, knowledge, experience, formal qualifications & training?

  • Have maintenance records/logs been kept for the plant and the relevant daily/weekly/monthly/yearly checks and services been carried out?

  • Were separate spotters/safety observers used to control mobile plant movement:

  • When operating in close proximity to each other?

  • When reversing?

  • Where the plant interacts with workers & others?

  • Did plant operators operate the plat at a safe speed?

  • Did plant operators operate the plant with the operator facing in the direction of travel?

  • Did the plant operators operate the plant in a forward direction wherever possible?

  • Did spotters maintain positive eye contact with the operator & are visible?

  • Did spotters maintain a safe distance from plant at all times?

  • Is plant painted in a distinctive bright colour?

  • Were audible reversing alarms used?

  • Were persons prevented from entering the operating working zone of mobile plant in which an operator is at the controls (whether the plant is in motion or not,) unless authorised to do so by the operator?
