EVALUATION

  • Are all activities performed in compliance with an accepted JSEA that is still adequate for the current tasks & conditions?

  • Do all persons appear to understand the requirements of the JSEA & have signed off?

  • Does the scaffold have a document attached to the structure (SCAFTAG for example) detailing the following:

  • Details of the person responsible for the structures' erection & subsequent inspections - for a scaffold structure of four metres height' or more (working platform height) by a qualified scaffolder?

  • The date the structure was last inspected - at intervals not exceeding 14 days by a qualified scaffolder for structures higher than four metres?

  • The structures' classification? (Heavy, medium, light)

  • Do all platforms higher than two metres have top rails, parallel mid rails & toe boards?

  • Are sole boards & baseplates used to properly distribute the load?

  • Are standards, joints, ledgers, transoms/putlogs & ties as per the scaffold plan?

  • Has a stair or ladder system been provided for access & egress?

  • Has mobile plant been re-routed away from the location of the scaffold to prevent collision & subsequent compromise to the structure, or barricades, signs, posts, buffer rails, guards or concrete or timber kerbs been used to prevent mobile plant from coming into contact with the scaffolding?

  • Have zones been implemented around overhead obstructions including power lines in which scaffolding has not been located?

  • Are scaffolds erected a safe distance away from trenches or excavations?

  • Are planks genuine scaffold planks in good condition, of uniform thickness (to prevent trip hazards) & secured against uplift?

  • Are platforms fully decked across their full width & free of gaps?

  • Are there any other risks or issues associated with the activities observed?
