Safety of areas where equipment installed

  • Is the area free of a obvious drainage problems ?

  • All equipment securely fixed unless it is designed to be portable ?

  • Is area free from any tripping hazards (tree roots, remains of equipment, fallen tree branches, uneven under surfacing ) ?

  • Are all borders secure and free of splinters ?

  • Is the compacted loose under surfacing at least (...) cm deep ?

  • Is the rubber/synthetic under surfacing adequate for the fall height of the equipment ?

  • Is the under surfacing under and around the the equipment, and extending at least fall zone (1.5m early childhood, 2.5m other ) from equipment base ?

  • Is fall zone free of rocks, stones, glass or other sharp objects ?

  • Was area free of other problems ?

Materials construction and installation of equipment

  • Is the equipment complete no missing parts ?

  • Are all timber joints and joints between timber and steel fastened properly ( all main bearing joints bolted ) ?

  • Are timber components free of excessive warping or splintering ?

  • Is all steel used for construction stainless or coated ( galvanised, powder coated, or painted ) ?

  • Are all end sections of tubular steel capped ?

  • Are all accessible bolt heads and nuts recessed, rounded or covered with protective caps and galvanised, coated or made from stainless steel ?

  • Are all small components including fasteners present ?

  • Are all components free from distortion ( eg. Not bent, vandalised or out of level ) ?

  • Is protective paint ( timber and steel ) or coating in good condition ?

  • Is all steel chain and it's connectors galvanised or made from corrosion resistant metal ?

  • Is equipment free from excessive rust, or evidence of broken members or cracked welds ?

  • Are all plastic, fibreglass components free of cracks, damage or wear that would effect their durability or safe use ?

  • Are all tyres ( whether set and fixed on to the ground or used in equipment ) non-steel belted types ?

  • Are all tyres free from broken or exposed rim wires or other protruding reinforcing wires ?

  • Are tyres bolted using large non-corrosive washers ?

  • Do tyres used above ground have drain holes and are these effective ?

  • Are concrete elements stable ?

  • Are all fixed structures ( including footings ) stable and do not deflect by the loading or movement of the user ?

  • Ae all footings and foundations covered with compacted soil so they do not protrude above ground ?

  • Are guard rails and hand rails secure ?

  • Ae shackles and wire ropes operating smoothly and lubricated if necessary ?

  • We rope nets sound including attachment and anchor points ?

  • Are sandpits free from unwelcome debris ( eg. Syringes, glass, animal droppings ) ?

  • Was area free from other problems during the audit ?

Head or neck entrapment hazards

  • Are accessible spaces over 600mm from ground level ?

  • Are these spaces greater than 125mm ?

  • Are spaces greater than 125mm also greater than 230mm ?
