  • DUCTS

  • Correct quantity and size of ducts were placed

  • Correct Footage Reported

  • Duct installed at correct depth

  • Ducts have muletape inside and secured at both ends of duct

  • Duct has tracer wire run outside and along the duct path or inside

  • Fiber warning posts installed approx every 500 ft

  • Trench path and any disturbance has been restored properly

  • VAULTS

  • Vault set with top level to and matching existing grade

  • Ground around the vault has been properly backfilled, compacted and seeded

  • Vault has a minimum of 6" of tamped gravel base

  • Vault (if splice location) has 5/8" X 8ft ground rod driven into the ground

  • Ducts are sealed at both ends and muletape secured and ground wire visible

  • Ducts not excessively protruding into the vault and layout prevents kinking, etc

  • Slack loop / splice case are properly secured, mounted, and labeled

  • Fiber min / max bend radius is not exceeded

  • Lid is seated properly latched or secured with bolts

  • Fiber warning post placed at vault location (not required at flowerpots)

  • SPLICE CLOSURES/OFDC's:

  • Cables / drops properly secured

  • Cables have color tape identification for feed / distribution

  • Cables have ID tags and at least one per case has Lit Fiber cable wrap.

  • Closure / OFDC is labeled with Vetro / Vitruvi ID number (ex: SC-00007412)

  • All entrance ports sealed and no missing plugs

  • Splice closures are properly bonded / grounded.

  • Splice closure or OFDC is properly mounted to strand / pole / vault.

  • Cables / drops are secured, neatly organized and not kinked.

  • Areas observed that require additional restoration
