LSR-Control of Work

  • Are Life Saving Rules available?

  • Is there a written JSA available for each task?

  • Are hazards, including hand and finger hazards, adequately identified?

  • Are mitigations identified for each hazard listed?

  • Is there an emergency plan/procedure in place with emergency contact information readily available?

  • Are permits in place for all work requiring a permit?

LSR-Isolations

LSR-Lifting

  • Are straps, slings, etc., in safe working conditions and stored properly?

  • Were tag lines or other tools that help manage a load used properly?

  • Is the operator of the lifting equipment certified and fit for duty?

  • Has the load weight been assessed to ensure the proper lifting equipment is used?

  • Is there a trained spotter assisting with the lift and transport of the load?

  • Are there barriers in place to prevent anyone from walking under a suspended load?

LSR-Driving

  • Is vehicle clean and orderly?

  • Does the driver have a valid driver's license and is there a current insurance card on hand?

  • Are emergency tools such as a fire extinguisher and first aid kit available and up to date?

  • Are loose objects secure to prevent injury in case of abrupt stops?

LSR-Working at Height

  • Is fall arrest equipment such as full body harnesses and lanyards in safe working condition and properly stored?

  • If a manlift is being utilized, is the proper documentation in place such as a manlift JSA, equipment inspection form, and rescue plan?

  • Are employees working at height properly tied off at 100% and using appropriate anchor points?

  • Are appropriate barriers in place to minimize injuries caused by dropped objects?

LSR-Confined Space

  • Has the confined space been properly identified (permit required or non-permit required)?

  • Are employees involved in confined space entry properly trained?

  • If required, is there a permit in place and are all the requirements being satisfied?

LSR-Ground Works

  • Is an excavation permit in place and are its requirements being satisfied?

  • Are located properly marked?

  • Is the Wood Group Ground Disturbance Checklist utilized to ensure all risks have been assessed thoroughly?

  • Are heavy equipment operators certified?

LSR-Fit to Work

  • Do employees appear rested as well as physically and mentally prepared for work?

  • Are employees properly trained in the tasks that are being performed?

General Safety

  • If hot work is being performed within 75 feet of a hydrocarbon source, is there a permit in place and requirements being met?

  • Are fire extinguishers readily available in hot work areas?

  • If respiratory protection is required to be worn, are employees current on their annual fit test?

  • Is respiratory equipment properly stored and in safe working condition?

  • Are the correct tools being used properly for each task?

  • Are employees taking into account hand and finger hazards by using appropriate gloves and using safe hand placement?

  • Is the work area free of slip and trip hazards or have they been mitigated?
