  • Use this audit to assess the following aspects of this site as Safe, At Risk or Not Applicable. Unsafe response indicate that attention is required to bring items to a satisfactory standard. Notes and photos should be attached where appropriate, particularly to At Risk items.

Fire Prevention

  • Is there an 18” clearance maintained between sprinkler heads and the top of stock storage. An 18” clearance is important to ensure uniform water contact with fire and better water dispersal.

  • Do sprinkler heads appear to be in good condition? Heads should not be painted, capped, etc.

  • Fire extinguishers have current tags? Gauge is in the green? Pin for handle is in place?

Exits

  • Do all emergency exit doors have a lighted sign?

  • Are emergency exits unobstructed?

  • Are emergency exit doors working properly and alarmed? If they are difficult to open that can impede egress. If they are not alarmed it could be a path for others to be let in without checking in at the front.

Walking Surfaces

  • Is there standing water anywhere?

  • Are there any trip hazards? Torn turf, loose cords, large divots in flooring, etc.

  • "Wet Area" cones readily available and/or in use if there is a wet floor?

Lighting

  • Is all inside lighting operational? If 1 or 2 light bulbs are out is there still adequate lighting?

  • Is emergency lighting tested on a monthly basis? There should be a small red test button on several light fixtures throughout the facility.

  • Does the location have an emergency flashlight?

Maintenance & Storage

  • Do all electrical panels have 36" of clearance on all sides?

  • Open electrical panel. Make sure there are no empty ports and that all switches are labeled

  • Equipment and supplies are stored on racks so that the lightest items are on top? This reduce falling concerns and prevents back injuries.

  • Storage areas are tidy?

  • All chemicals are labeled?

  • Personal protective equipment is available? Gloves, safety glasses

AED and First Aid

  • Automatic External Defibrillator (AED) is in working order? Flashing green light is on

  • AED cabinet is in good condition?

  • First aid cabinet is stocked?

  • Team members have CPR/AED certification?

Pool/Wet area and saunas

  • Pool / Spa rules are posted? No lifeguard on duty, emergency phone number, etc.

  • Pool decks and grates are in good condition?

  • ADA chair lift is operational?

  • Pool pump room is tidy?

  • Acid stored separately from chlorine and all liquids have secondary containment?

  • Water test log is up to date and chemical levels are within range? Proper chemical range sheet should be posted or easily accessible in binder.

  • Eyewash is in good condition? For plumbed test flow and make sure faucets are capped when done. For self contained make sure it is not expired.

  • Dry sauna checklist is completed every 30 minutes?

Locker rooms

  • Locker room is clean and free from hazards?

  • ADA stall and shower are in working condition? Bench is not broken, hand held shower is within reach of bench.

  • Theft prevention signs are posted?

Outdoors

  • Any graffiti?

  • Trash enclosures are tidy and secure?

  • Light is good after dark? If there during daylight ask employees to comment.

  • Any trip and fall concerns on sidewalk or parking lots?

Other safety, security, and compliance items

  • Security cameras are in working order?

  • Safety binder is readily accessible at the front of the club?

  • Recent safety topics are in the safety binder?

  • Clothes dryer lint trap is clean?

  • Labor law poster and Nurse Triage poster is readily visible in employee break area?

General Comments

  • Any further comments or recommendations arising from this inspection?

  • Add any additional relevant photos

  • Have all required corrective actions been added as Actions to this inspection?
