INSTRUCTIONS AND COMMENT

  • 1) This form will be used by the site responsible supervisor or manager to carry out an audit of the behaviour of workers, employees and contractors in respect to Health & Safety issues & behaviours. 2) Each behaviour audit shall last a minimum of one hour of site observation and all findings shall be recorded on this audit form and where applicable added to Vault

  • Correct PPE being worn as per site requirements?

  • Pedestrians are keeping to walkways. No - one seen taking short-cuts?

  • Pedestrian interaction rules being followed? - No pedestrians are within 2 metres of working MHE.

  • Forklifts driving rearwards where visibility is impaired?

  • Vehicles not operating too fast or driving dangerously ( inc Drivers not using phones ) ?

  • Forklift operators wearing seat belts?

  • Forklift checklists carried out - request evidence ?

  • Isolation and lockout correctly followed where applicable ?

  • Permanent site workers including employees and labour hire are inducted ( Challenge at least 4 ) ?

  • All contractors on site are inducted ?

  • Applicable licences are available such as Forklift, Electrical, Fitter , High Risk ?

  • Applicable permits filled out and authorised such as hot work and confined space ?

  • Safe manual handling practices taking place ?
