Amenities

  • Demarcated exclusion zones with signage is in place.

  • Correct PPE is being worn.

  • Arrangement for safe disposal and transport to waste facilities are in place.

  • Chemical containers are labelled and stored in bunded areas or pallets.

  • Gas bottles are stored upright and secure.

  • Flammable gas bottles are stored in ventilated areas separated from ignition sources.

Lighting

  • Adequate lighting (Walkways, Site-shed, Storage.

Worksite protection

  • Worksite protection plan is on site and current <br>NOTE<br><br>Implementing protection in the Danger Zone must be completed by a protection officer.

Traffic Management

  • Onsite traffic management plan is in place

Electrical

  • RCDs (Earth Leakage Protection) are in use for all leads.

  • All portable electrical tools, leads and equipment are in good condition and are tested and tagged

Safe Approach Distances

  • Safe approach distances / Electrical permit are maintained

  • Arrangements are in place to prevent encroachment.

  • Mobile plant operator(s) and safety observer hold 'Safe Electrical Approach for Crane and Plant' Competency.

Plant

  • Plant and equipment register (Including electrical) is available.

  • Log books and inspection records are available for all plant.

  • Plant operator has the required qualifications (E.g Current High Risk Licence).

  • Operational flashing lights, reversing siren is available and in good working order for all mobile plant.

  • Daily Pre-Start checks have been completed for all plant in use.

  • Exclusion zones are in place around mobile plant.

Hot works

  • Hot works permit is completed.

  • Work and requirements comply with permit.

  • Cylinders stored fixed working upright

  • Grinding / Welding is protected ( Face shield, Fire Extinguisher, Spark screen and correct PPE

Working & Heights

  • Fall prevention is in place with a rescue plan.

  • Fall prevention System (E.g Harnesses, Lanyard, Static line) is in place and inspected as required by legislation.

  • Areas below personnel working at heights is barricaded.

Ladders

  • Ladders are used for access only.

  • Ladder extends at least 1m above landings at an angle of 1:4 and the top and bottom are lashed.

  • Ladders are non-metallic in the rail corridor.

Excavation

  • Excavation plan has been approved and is being followed.

  • Asset owners are consulted when excavating in the vicinity of services with DYBD and areas are marked.

  • No mechanical excavation is being undertaken within 2m of known services.

  • Excavations are Benched, Battered, Shored and edge protection is in place.

Scaffolding

  • Scaffolding is certified with regular inspection and Scaff Tag in place on all entry points to the scaffold

  • Scaffold is complete with handrails, Kick-boards and internal/External Access.

  • Solid foundation under scaffolding and secured against vandalism.

  • Area around scaffold is barricaded.

  • Wheels are locked on mobile scaffold.

Demolition

  • Demolition plans are in place and qualified supervisor is on site.

Formwork

  • Engineers formwork certificate is available(As Applicable) and complies with AS3610.

  • Sole plates are in use.

  • Secure access is supplied.

  • Area barricaded during erection of formwork

Confined Space

  • Confined space permit is in place and correctly filled in.

  • All personnel hold Confined Space Certification and training.

  • Confined space watcher permit is in place with Gas reading sign on/off by all.

  • Rescue plan is in place.

Environmental

  • Noise issues are managed.

  • Dust is minimised.

  • Water run-off is controlled.

  • Sedimentation barriers are in place Spill kits are available on site and are stocked correctly.
