Work at Height

  • Have lifelines been installed and anchored to secure fixed points as an initial activity?

  • Is there is a safe method of getting to and from work area?

  • Is there protection in place or procedure for materials shifting or during scaffold work to protect scaffolders or nearby workers from falling materials?

  • Have personal fall arrest systems been provided for all workers where necessary?

  • Is scaffolders safe zone being implemented or is there a safe temporary platform for the scaffolders?

  • Have fixed ladders provided to give safe access to all working platforms?

  • Are safety harnesses worn by all persons working at height and they securely fixed to a fixed point or life line at all times to stop their falling?

  • Are shock absorbing lanyards used in all cases?

  • Are all tools used while working at height such as spanners, wrench, level etc., has tool lanyard to avoid it from falling down?

  • Does fall protection equipment in good condition and do not show any damaged? This is confirmed by site in-charged?

Scaffolds

  • Are scaffolds erected, altered and dismantled by competent person?

  • Are all uprights provided with base plates (and, where necessary, timber sole plates?)

  • Are all uprights, ledgers, braces and struts in position?

  • Are boards tied down/secured?

  • Are boards lapped over others? If this cannot be avoided are they safe from tripping hazards?

  • All scaffold to have proper ladder access within the scaffolding.

  • Is the scaffold secured to the building or structure in enough places to prevent collapse?

  • Are there double guard rails and toe boards, or other suitable protection, at every edge, to prevent falling?

  • Are scaffold boards in good condition?

  • Adequate cross bracing provided.

  • Are there effective barriers or warning notices in place to stop people from using an incomplete scaffold, avoid alteration and /or PPE requirement, e.g. where working platforms are not fully boarded?

  • Is there proper barrication and warning notice in place to stop people from unathorized entry within the working area?

  • Does a competent person inspect the scaffold regularly, e.g. at least once a week; and always after it has been altered or damaged and following extreme weather?

  • Are the results of inspections recorded?

  • Scaffolds periodically inspected for sound condition using scaffolding tag system.

Housekeeping

  • Maintain general neatness of working areas.

  • Walking & working area to be clear, tidy, and free of debris & tripping hazards?

  • Maintain storage clean & materials properly stacked?

  • Protect access & exit points from work overhead.

Personal Protection Equipment

  • Does the user wearing adequate/ necessary PPE for the task?

  • PPE (Helmet, Safety shoes, Jacket, Gloves, Safey goggles) available & utelized in the project.
