Building Entrances

Stairs and Ramps

  • Are step nosings hard to see, rounded, damaged or slippery? 

  • Are handrails available?  

  • Are they easy to reach and usable?

  • Is the height of the steps or depth of tread inconsistent?

Work Areas and Work Platforms

  • Is poor drainage causing a pooling of liquids or leading to poor traction?

  • Are designated walkways usable and free of materials?

  • Are walkways uneven, do they have holes or missing pieces of concrete?

  • Are there any trip hazards around work areas and equipment?

  • Are light levels too low to see clearly?

  • Is light bouncing off equipment creating a glare

Behavior Observation

  • Are employees or contractors performing at risk behaviors (i.e., climbing over piping or protection, taking shortcuts not wearing proper fall protection, etc)?

  • Do tasks stop employees or contractors from seeing slip/trip/fall hazards (i.e. carrying items that restrict view, upset people's balance, rushing, etc.)?
