    This checklist shall be completed for all past and current staff employed in the care home. This checklist together with all back up documents as per checklist shall be filed in the individual staff file

  • Evidence of date of birth? ( obtain copy of birth certificate, or if not available then copy of passport. Any other form of evidence considered

  • Evidence for right to work in the UK

  • Documentary evidence of qualifications obtained

  • Does driving license include any driving convictions

  • Evidence that person employed is physically and mentally fit to complete job?<br><br>Is declaration form C4006 and C4007 on file?
